Eccema:

Prevencin primaria
Actualmente no existe evidencia para una dieta de eliminacin o el uso de
probiticos durante el embarazo y/o lactancia.
La introduccin tarda recomendada por la AAP en el ao 2000 result en un
aumento en la prevalencia de alergia alimentaria.
No hay suficiente evidencia de que las frmulas hidrolizadas o de soya
prevengan la alergia a la protena de leche de vaca.

El tratamiento de eleccin al diagnosticar alergia al a leche de vaca (D)
Se deben dar instrucciones verbales y por escrito incluyendo lacteos y
aquellos con ingredientes ocultos
Debe adaptarse a la edad del nio y debe incluir a sus cuidadores
Se recomienda una evaluacin cada 6-12 meses para valorar la
reintroduccin de la leche.

La leche de vaca se encuentra en una gran variedad de alimentos y es de los
alrgenos mas difciles de evitar.
Los consumidores pueden esperar la leche como ingrediente en algunos
alimentos; sin embargo, otros requieren conocimiento mas especializado
para su deteccin.
Desde 2005 la Unin Europea orden que los alimentos empaquetados
indicaran la presencia de 14 alrgenos, entre ellos leche de vaca.

La leche es fuente de protena, calcio, fsforo, riboflavina, tiamina, vitamina A
y B12.
Los nios en lactancia materna y mayores de 2 aos no necesariamente

Se reporta que en el 80% de los casos, se adquiere tolerancia a los 3-4
aos de edad.
EuroPrevall: tolerancia de 2/3 de los pacientes al ao del diagnstico.
Las patologas no mediadas por IgE tienden a tener una resolucin
mas temprana.
Influyen factores como antecedentes familiares, comorbilidades
alrgicas, cosensibilizaciones, alrgenos involucrados.

Conclusiones personales
La alergia a la protena de leche de vaca es un tema de gran inters
que requiere conocimiento experto en una variedad de areas para la
atencin adecuada.
Debe hacerse un diagnstico certero y oportuno debido a la
importancia de la dieta de eliminacin como tratamiento.
Se debe estar familiarizado con las opciones alternativas de
alimentacin, as como suplementacin, y alternativas no aptas.
