AFFIDAVIT

I, CHRISTIAN JOSEPH G. REYES, of legal age, Filipino citizen, single,

presently reside at Brgy. 20, Basa St. Laoag City, Ilocos Norte, after having been
duly sworn to in accordance with law, do hereby depose and say that:

1. I am the son of Mr. Ernesto A. Reyes and Mrs. Gloria Tomasa G.

Reyes, both deceased;
2. My mother Mrs. Gloria Tomasa G. Reyes was a member-
consumer of Ilocos Norte Electric Cooperative (INEC);
3. I am the only son and legal heir of Mrs. Gloria Tomasa G. Reyes;
4. I am executing this affidavit to attest to the truth of the foregoing
statements given above and as requirement for financial aid
granted by INEC for its deceased member-consumer;
5. I am always ready and willing to testify as to the veracity of the
above stated facts anytime when called upon to do so.

IN WITNESS WHEREOF, we have hereunto set our hands this 30th day
