AFFIDAVIT

I, CRISTIAN LEE R. RAMIREZ, of legal age, single,

resident of #20, San Antonio Village, Matina, Davao City,
after being sworn in accordance with law, depose and say:
1. That on October 18, 2017, 9:12 AM while at home, I
listened to the radio station Win Radio Davao
107.5Mhz, advertising the product HEMROID HERBAL
SUPPLEMENT CAPSULE;

2. That the following statements were made:

Grabe jud kaayo ang gibati nako na sakit kaniadto,
dili na jud maantos kay grabe na jud inig kalibang nako
mi-dugo unya sakit kaayo. Mao to gipa-konsulta sa
doktor. Niingon ang doktor nga operahan ko, unya wala
man koy kwarta pa-opera! Nag-ampo ra jud ko nga naa
ra jud makatabang nako. Mao to naa ko nadunggan sa
radyo nga Hemroid tablet. So nipalit ko, gi-take dayun
nako tapos pila lang ka adlaw naayo na siya.
Nagpasalamat kaayo ko dako ana kay naa bitaw tambal
nga ing-ani para bitaw sa mga walay kwarta. Salamat
kaayo sa Hemroid tablet, salamat kay naayo jud akong
almoranas.

3. That the said tablet claimed to cure hemorrhoid when

taken; and

4. That no Disclaimer was made immediately after the

advertisement.
