ASUNTO : Remito Conformidad de Pago de Asistente Tcnico de Obra correspondiente al

mes de febrero y marzo 2017.

NOMBRE DEL PROYECTO : "MEJORAMIENTO DE LA LOSA MULTIDEPORTIVA DEL

CENTRO POBLADO DE CHAHUARMA, DISTRITO DE LIRCAY,
PROVINCIA DE ANGARAES - HUANCAVELICA"

Es oportuno dirigirme a su digno despacho con la finalidad de reportar informacin referente al servicio realizado
en calidad de asistente tcnico del proyecto: "MEJORAMIENTO DE LA LOSA MULTIDEPORTIVA DEL
CENTRO POBLADO DE CHAHUARMA, DISTRITO DE LIRCAY, PROVINCIA DE ANGARAES -
HUANCAVELICA". Al haber cumplido las funciones de acuerdo a la Orden de Servicio N 000150 y tenindose el
INFORME N 0152017/MPAL/GM/GLyGT/RO-JPGA. Del Residente de obra. Por lo tanto, se otorga conformidad de
pago por el servicio prestado de acuerdo a la valorizacin fsica ejecutada correspondiente del mes de febrero (13.32%) y
Marzo (31.61%) con un total de avance 44.93% de valorizacin a favor del Sr: Gustavo Espeza Enrquez (Asistente
tcnico de Obra), identificado con DNI N 47671075 y RUC N 10476710753, por el importe de S/. 1,752.27 (Un mil

setecientos cincuenta y dos con 27/100 Soles).

Cualquier informacin adicional o ampliacin estar a disposicin de la Gerencia De infraestructura y gestin territorial
de la Municipalidad Provincial De Angaraes Lircay.
