REQUIREMENTS FOR RENUNCIATION OF PHILIPPINE CITIZENSHIP

1. Personal Appearance
2. Submit ALL Philippine passports (old and new) with 1 photocopy each (important
pages onlybio data page, last page, amendment page, if any)*
3. Accomplish Affidavit of Renunciation 2 original copies (form may be obtained from
the Office or may be downloaded from website)
4. Present the original of the following with 3 photocopies:
a. Philippine Statistics Authority (formerly NSO)-authenticated Birth Certificate (in
security paper)
b. Hong Kong ID
c. Letter from Hong Kong Immigration (for Chinese naturalization); or
Personal letter-request addressed to the Philippine Consulate General (for
Taiwanese visa or other procedure that require renunciation of PH citizenship)
d. Marriage Certificate (if married)
e. HK SAR Passport (for Taiwanese visa or other procedure that require
renunciation of PH citizenship)
5. Pay the corresponding processing fee HK$400 (HK$200 for notarization and HK$200
Certificate of Renunciation); Processing period is 2 working days
