TOPIC- Ocular prosthesis

Synopsis:Physical abnormalities or defects that compromise appearance and function are sufficient to
render an individual incapable of leading a relatively normal life.removal of any organ may
be indicated in case of severe trauma, congenital abnormality or disease such as an infection
or a tumour. this loss of organ has a traumatic effect on the psychology and social status of an
individual. Hence, in such situations, a maxillofacial prosthodontist plays a crucial role in
restoring the structure, function and cosmetics of the defect created.
Maxillofacial prosthesis ia an art and science of anatomic, functional or cosmetic
reconstruction by means of non living substitutes of those regions in maxilla, mandible and
face that are missing or defective because of congenital malformation, developmental,
trauma, pathology or due to any surgical intervention.
Early records indicate thal artificial eyes,ears and nose were found on Egyptian mummies.
Tycho Brahe, a Danish astronomer in the 16th century, lost his nose in a duel and had it
replaced with an artificial nose made of silver and gold. The London medical gazette of 1832
reported gunner with the silver mask, a French soldier, whose face was seriously injured in
a war. The left half of mandible was almost carried away. A physician desgined a prosthetic
restoration for him which looked like a mask. Pierre fauchard in 1728 used perforations of
palate to retain artificial dentures.
Kingsley (1880) described the artificial appliances used for the restoration of congenital as
well as acquired defects of the palate, nose and orbit. A definitive technique for fabricating
artificial eyes using acrylic resins were developed by united states of naval dental and
medical schools and was published in 1944. In the early 20th century, prosthetic restorations
were carried out through collaboration of dentists and plastic surgeons.
OBJECTIVES OF MAXILLOFACIAL
REHABILITATION
1. Restoration of esthetics or cosmetic appearance of the patient.
2. Restoration of function.
3. Protection of tissues.

4. Therapeutic or healing effect.


5. Psychological therapy.
The maxillofacial prosthetic approach has the following three main advantages:
1. It requires little or no surgery.
2. The patient spends less time away from home and job.
3. The reconstruction often has a more natural appearance.
The drawbacks of this approach include the following:
1. The necessity of fastening the appliance to the skin daily.
2. Removing the appliance daily.
3. The occasional need of constructing a new prosthesis
Sensory organs play a significant role in our daily lives and EYE is a vital organ not only in
terms of vision but also for facial expression. The most tragic yet, unfortunately, the most
commonly occurring loss of these sensory organs is that of the eves. Eves are generally the
first features of the face to be noticed and the presence of a pair of eyes is quite essential to
maintain the balance and the aesthetics of a face. The goal of any ocular prosthetic procedure
is to enable rehabilitation of the patient hi society with a normal appearance and reasonable
mobility of the prosthetic eve. The indications for the surgical removal of an eye are
irreparable trauma like bomb explosions, fights, infection, tumour, blindness, pain hi the eve,
the need for histological confirmation of a suspected diagnosis, possible prevention of
svmpathetic opthalmial and cosmetic reasons. Surgical procedures adopted for the removal of
an eve are classified into three general categories: evisceration, enucleation and exenteration.
Ocular prostheses can be given to patients who have lost ocular structures through orbital
evisceration or orbital enucleation.

LIMITATIONS:
1. Whose shapes were inaccurate and difficult to reproduce
2. Time consuming
3. Allergic to the patient
4. Labour intensive.

6) BRIEF RESUME OF INTENDED WORK:


a) INTRODUCTION:
Now a days implants are playing a major role in dental field due to their
efficiency and patient acceptance. The resultant crestal bone levels around dental implants
following restoration are used as a reference for evaluating the implant success.

The concept of platform switching refers to the use of smaller


diameter abutment on a larger diameter implant collar. This connection shifts the implantabutment junction towards the central axis of implants. It also repositions the inflammatory
cell infiltrate, confining it within a 90 degree area that is not directly adjacent to the crestal
bone, which limits the bone change that takes place around the coronal aspect. Hence, this
results in reduced bone loss.

The purpose of this study was to analyse and compare implant bone interface stresses in
three dimensional finite element model of an osseointegrated implant, with platform
switching and with conventional matching diameter implant platform and abutment in
different bone densities under different load applications in posterior mandible with zirconia
crown.

b) REVIEW OF LITERATURE:

1.

Xavier vela nebot et al in the year 2006 conducted a study to compare the rate of bone

resorption in conventional to that of platform switched implants. In this study the author
studied a series of 30 control cases with conventional implants and 30 cases of platform
modification technique. Interproximal bone resorption on mesial and distal surface of each
implant at 1,4 and 6 months was assessed using digital radiography. The author concluded
that the mean value of bone resorption on mesial and distal surface of two phase platform
switched implants is less than that of conventional implant system.
2.

Flemming isidor in the year 2006 has reviwed the effect of occlusal forces on oral

implants and the bone surrounding it. in this the author reviewed that occlusal forces has a
major effect on oral implants and its surrounding bone. According to bone physiologic
theories,bones adapt to additional stree subjected on them,which is bone remodeling. This
bone remodeling is a apposition, thus increasing bone density. Beyond

this limit, this

additional stress results in resorption of the bone. The author in this review concluded the
most possible causes for implant failure.

Luigi canullo in the year 2007 conducted a study to evaluate the effect of immediate

loading implants on soft and hard tissues. In this study, the author placed immediately loading
implants soon after extraction without compromised bone loss. Implants were with 6mm
diameter platform and 4mm diameter abutment and provisional crown was adapted. Three
months following implant placement, prosthetic rehabilitation was done. Pocket depth,
recession, papilla height and radiographic assessment was checked at the time of insertion.
The author concluded that all the 10 implants were osseointegrated. Software analysis
showed lesser bone resorption than normal. Even pocket depth was normal. Hence, he

concluded that immediate loading with platform switching provide peri implant hard tissue
stability with soft tissue and papilla preservation.

c) AIMS AND OBJECTIVES:


1] To evaluate the stress distribution pattern in conventional matching diameter implant
in different bone densities under different load applications.

2} To evaluate the stress distribution pattern in platform switched implant in different


bone densities under different load applications.

3] To compare the stress distribution in conventional matching diameter and , platform


switched implants in different bone densities under different load applications

7) MATERIALS AND METHODS

B] Method:
In this intended study, a FE model of mandibular posterior region with
different bone densities will be designed by incorporating Young modulus and Poissons ratio
using ANSYS software for finite element analysis. Implants with conventional matching
diameter abutments and platform switched abutments will be designed and subjected to
different vertical and oblique loads. The stress distribution pattern will be evaluated and
compared.
