AFFIDAVIT
I, JESUSA CORTEZ DE LEON, of legal age, Filipino, married with residence
at #7127 P. Burgos St., San Dionisio Paranaque City, after having been duly
sworn to in accordance with law, hereby depose and say concerning my
deceased grandmother, - that:
1. I am one of the grandchildren of CONCORDIA M. DE LEON, who died
intestate on August 1, 2014 in Paranaque City.
2. That I am only the nearest relative to be present in the cremation of
her remains to be serviced by the Manila Memorial Park.
3. That as the only representative of my family I hereby give my consent
in behalf of my mother to the cremation of my said grandmother in the
absence of my mother.
4. That I and my family hereby hold the Manila Memorial Park, all its
directors, representative, employees of any responsibility and liability
for the cremation to be made on her remains.
5. That in case there will be any case that would be filed arising in this
cremation, our family shall be the one who would be responsible and
not the Manila Memorial Park.
6. I am executing this affidavit to attest to the truth of all the foregoing
facts and for all legal intents, effects, consequences and purposes.
