a. All employees have been informed and will be transferred and shall in no way
constitute unfair labor practice nor prejudice labor rights;

that were lodged, description thereof, number of employees concerned,
status and statement that ____ shall comply with the final resolution on said
case/s;
c. certifies that the list of all existing equipment, assets and other goods in
____s Old Site to be transferred to the New Site and those to be retained at
the Old Site, if any, submitted by ___ to PEZA in compliance with PEZA
requirements for the transfer of its operations from Old Site to New Site,
comprise 100% of the assets / equipment of ____ at the Old Site acquired
with PEZA incentives and that ___ has not disposed of any goods/assets
acquired with PEZA incentives without the required PEZA permits.
____ shall secure from the PEZA-Building Official at the ____________ the
necessary Occupancy Permit, Permit to Operate and other permits under the
National Building Code of the Philippines for the construction/fit-outs of its
facility/building in the New Site and installation of equipment therein. (as
applicable, for the partitions required under this authority to segregate the
operations of ___ from ____ in the above-subject building space to be leased out
to ____ for its non-PEZA operations);

___ shall pay to PEZA all corresponding fees and dues related to the issuance of
the said Permits as specified in the NBCP and its implementing rules and
regulations. Annual NBCP permits shall be secured from the PEZA-Building
Official at the _________;
The transfer of goods from the Old Site to the New Site shall be subject to the Zone
Administrator/Manager/Officer-In-Charges issuance of the appropriate PEZA form
under standard PEZA rules and procedures in the movement of goods subject to
compliance to the above-listed requirements; submission of liquidation ledger on the
permanent transfer of all goods to the New Site and to other requirements as maybe
prescribed the PEZA Zone Administrator/Manager/Officer-In-Charge;

All transfers of machinery, equipment, supplies, and other assets from ___s Old Site to
its New Site shall be covered by PEZA Permit Form 8106 to be issued by the PEZA
Zone Administrator/Manager/Officer-In-Charge prior to every transfer and to a surety
bond to cover the duties and taxes for said machinery/equipment and other
assets/goods to be transferred to the New Site.
The Surety Bond no., O.R. No., amount and date shall be indicated on the PEZA
Permit form 8106 and a copy of the said bond and Official Receipt validated by
the bonding company shall be presented to the PEZA Zone
Administrator/Manager/Officer-In-Charge together with the original copy of the
Official Receipt.

Prior to ___s first importation / exportation under PEZA incentives in the New Site, ___
shall:
a) submit to the PEZA Zone Administrator/Manager/Officer-In-Charge of the
New Site, clearance from the PEZA Zone Administrator/Manager/OfficerIn-Charge of the Old Site stating that the ___s existing accounts with all
PEZA accredited Value Added Service Providers (VASPs) for the PEZA
e-permits registered under the Old Site have been deactivated, including
its Client Profile Registration System (CPRS) with the Bureau of Customs
(BOC) or as applicable, the schedule of the deactivation of all accounts
under PEZA and the BOC;
b) register with the BOC a CPRS as importer / exporter, for the New Site,
through any or all of the PEZA-accredited VASPs;
This authority shall be valid for three (3) months on the date of this approval. Any
extension
hereof
shall
be
issued
by
the
PEZA
____
Zone
Administrator/Manager/Officer-In-Charge, subject to the same terms and conditions of
this LOA.

any liability/claim or any damages that ___ may suffer in the event of

Center/ Park/Building or is not proclaimed by the President as a PEZA

any tax issues or Permit which the Local Government of ____ may raise in
view of ____s building renovation/fit-outs and installation of machineries
at the ____________ (New Site) prior to its Presidential Proclamation as
a PEZA Zone / IT Center / Park / Building;
d. acknowledges that:

any assets which will be retained in the _______ (Old Site), once all of
___s operations have transferred in the ____________ (New Site) shall
be subject to the payment to the Bureau of Customs (BOC) of the
corresponding duties and taxes.
e. shall retain an office in the Old Site until the New site is proclaimed by the
President as a PEZA Zone/IT Center / Park / Building and the Developer /
Operator hereof has signed its Registration Agreement with PEZA; and
f. certifies that the list of all existing equipment, assets and other goods in
____s _____________ (Old Site) to be transferred to the ______________
(New Site) and those to be retained at the _________(Old Site), if any,
submitted by ___ to PEZA in compliance with PEZA requirements for the
transfer of its operations from __________Old Site to __________New Site,
comprise 100% of the assets / equipment of ____ at the ________Old Site
acquired with PEZA incentives and that ___ has not disposed of any
goods/assets acquired with PEZA incentives without the required PEZA
permits.
g. _____ shall inform PEZA in writing of the date of its start of operations at
___________ (New Site) within seven (7) days from said date.

All Permits required under the National Building Code of the Philippines (NBP)
for said construction shall be secured by ____ from the local governments
Building Official, until ______________ is proclaimed as PEZA Zone/IT
Park/Center/Building; and said Permits shall be presented to the PEZA Building
official prior to the registration with PEZA of the _____s ____________ facility;

Upon registration of the _____________ New Site with PEZA, _____ shall
secure from the PEZA-Building Official at the ____________ the necessary
Occupancy Permit, Permit to Operate and other permits under the National
Building Code of the Philippines for the construction/fit-outs of its facility/building
in the New Site and installation of equipment therein. (as applicable, for the
partitions required under this authority to segregate the operations of ___ from
____ in the above-subject building space to be leased out to ____ for its nonPEZA operations);
___ shall pay to PEZA all corresponding fees and dues related to the issuance of
the said Permits as specified in the NBCP and its implementing rules and
regulations. Annual NBCP permits shall be secured from the PEZA-Building

____s importation / local procurement of equipment and other goods intended for the
New Site shall be subject to applicable standard procedures. All PEZA permits / Import
Permit (eIP) / Permit to Bring Goods from the Customs Territory to the PEZA Zone/IT
Park Center/Building (PEZA Form 8105 Permit) / invoices shall be made out to / shall be
issued to ____s operation at its Old site facility;

All transfers of machinery, equipment, supplies, and other assets from ___s Old Site to
its New Site shall be covered by PEZA Permit Form 8106 to be issued by the PEZA
Zone Administrator/Manager/Officer-In-Charge prior to every transfer and to a surety
bond to cover the duties and taxes for said machinery/equipment and other
assets/goods to be transferred to the New Site; The bond shall be deemed liquidated
only upon Presidential Proclamation of the New Site as a PEZA Zone / IT Center / Park /
Building;
The Surety Bond no., O.R. No., amount and date shall be indicated on the PEZA
Permit Form 8106 and a copy of the said bond and Official Receipt validated by
the bonding company shall be presented to the PEZA Zone
Administrator/Manager/Officer-In-Charge together with the original copy of the
Official Receipt.
The transfer of goods from the Old Site to the New Site shall be subject to the
Zone Administrator/Manager/Officer-In-Charges issuance of the appropriate
PEZA form under standard PEZA rules and procedures in the movement of
goods subject to compliance to the above-listed requirements; submission of
liquidation ledger on the transfer of all goods to the New Site and to other
requirements
as
maybe
prescribed
the
PEZA
Zone
Administrator/Manager/Officer-In-Charge;
11.
Upon arrival of the assets / goods at the New Site, ____ shall submit the said list of
assets / goods to PEZA Zone Administrator/Manager/Officer-In-Charge for inspection to
ensure that the same have been delivered at the New Site; on the basis of which the
corresponding PEZA Form 8106 Permit shall be marked by the PEZA official with the
following: Inspected at __(floor /building no.)___ of the New Site;
12.
Any and all of ____s assets, equipment, whether imported or locally purchased, fit-outs
shall not be moved out of ____s facility at the New Site, unless a Letter of authority
from PEZA is obtained by ____ prior to movement of said goods;
13.
Prior to the approval of the last PEZA Form 8106 Permit for said transfer from the Old
Site to the New Site, ___ shall submit the following to the PEZA Zone
Administrator/Manager/Officer-In-Charge:
a. Application for PEZA Permit Form 8105 to which shall be attached a certified
list of all assets, equipment and other goods transferred from the Old Site to
the New site duly signed by ____s highest responsible official; The PEZA
Form 8105 Permit shall be approved / signed by the PEZA Zone
Administrator/ Manager /Officer-In-Charge upon Presidential Proclamation of
the New Site as PEZA Zone / IT Center / Park / Building and the Developer /
Operator signed the corresponding Registration / Supplemental Agreement
with PEZA;
b. Official receipt from the BOC for ____s payment of duties and taxes on
imported goods and/or VAT on locally procured goods retained at the Old
Site, if applicable. For locally procured on which VAT is paid, ____ shall file
the corresponding VAT returns with the Bureau of Internal Revenue (BI) to
report said VAT payment, if applicable;
c. Proof of receipt of the PEZA Office of the Board Secretary (OBS) of the
original copy of ____s Certificate of PEZA Registration for amendment of
____s location to the New Site; The PEZA-OBS shall release to ___ its
amended PEZA Certificate of Registration only upon Presidential
Proclamation of the New Site as a PEZA Zone / IT Center / Park / Building,
the Developer / Operator hereof has signed its Registration Agreement with
PEZA and the ___s submission of a clearance from the PEZA _____ Zone
Administrator/Manager/Officer-In-Charge that ____ has complied with all the
requirements for the transfer of operations;

____ shall abide by/comply with the statements in the above-cited notarized
undertaking in condition no. 5c. ;
15.
Only upon Presidential Proclamation of the New Site and the Developer / Operator
hereof has signed its Registration Agreement with PEZA, ____s first importation /
exportation under PEZA incentives at the New Site, ___ shall be subject to ____s:
a) submission to the PEZA Zone Administrator/Manager/Officer-In-Charge of
the
New
Site,
clearance
from
the
PEZA
Zone
Administrator/Manager/Officer-In-Charge of the Old Site stating that the
___s existing accounts with all PEZA accredited Value Added Service
Providers (VASPs) for the PEZA e-permits registered under the Old Site
have been deactivated, including its Client Profile Registration System

of the deactivation of all accounts under PEZA and the BOC;

As applicable, ___ shall submit to the PEZA Enterprise Registration
Division proof of its application with the Bureau of Internal Revenue (BIR)
for Branch Code for its New Site (would specifically refer to an enterprise
with other existing locations other than the Old Site);
c) submission to the PEZA Zone Administrator/Manager/Officer-In-Charge of
the New Site, of its list of importables (goods/items to be imported /
exported that are entitled to tax and duty exemption) and/or exportables
(goods/items
as
authorized
to
be
exported
in
its
Registration/Supplemental Agreement/s or Letter of Authority), for
approval in the PEZA E-IPS, and E-AEDS.
Miscellaneous Requirements
16.
This authority shall be valid for three (3) months on the date of this approval.

____ shall not engage in any other operations in the said site. Any change in ____s use of the
above-subject premises shall be subject to prior PEZA clearance/approval;

This authority shall be valid for three (3) months on the date of this approval;

Any request for renewal of this Letter of Authority shall be subject to approval/issuance of a new
Letter of Authority by PEZA under the same terms and conditions of this Letter of Authority,
subject
further
to

a)

recruitment hub, including rentals, utilities and services.
b) proof of payment of VAT on locally procured goods for its recruitment hub, including
rentals, utilities and services.

In no case shall ____s employees, goods, machinery and equipment for its
PEZA-registered manufacturing/warehousing operations at its building facility
be co-mingled with the employees, goods, machinery and equipment of
____(NON-PEZA REGISTERED ENTERPRISE)___, and that for this purpose
___ shall provide the PEZA ____ Zone Administrator/Manager/Office-InCharge with such information and documents as may be necessary to ensure
that no such co-mingling of ____s employees, goods, equipment and

No tax and duty exempt or VAT-zero rated goods of ___ shall be brought to
the facility of ____(NON-PEZA REGISTERED ENTERPRISE)___;

Failure by ____ to submit the above requirements to the PEZA ____ Zone
Administrator/Manager/Officer-In-Charge shall be sufficient cause for PEZA ____
Zone Administrator/Manager/Officer-In-Charge to withhold issuance of any
authority/PEZA permit to ____.
2. ____ shall comply with and shall be bound by its undertakings provided in Condition
No. 1 (b) of this Letter of Authority.
3. ____ shall secure from the PEZA-Building Official at the ______________ the
necessary Occupancy Permit, Permit to Operate and other permits under the
National Building Code of the Philippines for the partitions required under this
authority to segregate the operations of ___ from ___(NON PEZA REGISTERED
ENTERPRISE)___ in the above-subject building space to be leased out to ___(NON
PEZA REGISTERED ENTERPRISE)___ for its non-PEZA operations.
___ shall pay to PEZA all corresponding fees and dues related to the issuance of the
said Permits as specified in the NBCP and its implementing rules and regulations.
Annual NBCP permits shall be secured from the PEZA-Building Official at the
_________;
4. ____ shall pay a franchise fee to PEZA in the amount of Php 12,000 on or before the
5th day of January of each year, without necessity of demand, as long as it is leasing
any portion of its building facility to ___(NON PEZA REGISTERED
ENTERPRISE)___. In case of delinquency of payment thereof, such delinquency
shall bear interest at the compounded monthly rate of two percent (2%) per month
computed from the date of delinquency.
5. Income realized from rental of said area and other related services provided by
___(NON PEZA REGISTERED ENTERPRISE)___ to ____, shall be
considered/reflected as Other Income in ____s book of accounts, Income Tax
Returns and Audited Financial Statements, and shall be subject to payment of the
Regular Corporate Income Tax. For this purpose, ____ shall set up a separate
account for the revenues/income from its leasing activity, including related services
provided to ___(NON PEZA REGISTERED ENTERPRISE)___ to segregate it from
income earned from its PEZA-registered operations and shall further be reported as
a separate item in its monthly and annual reports on operations submitted to the
PEZA-Enterprise Regulations Department in accordance with Rule XXI, Section 4 of
the PEZA Rules.
6. AII shall submit such documents/information as PEZA may require in relation to its
leasing operations.
7. Any additional lease of other areas in ___s building facility shall be subject to prior
PEZA approval.
8. This authority shall be valid until ___________. Any request for renewal of this
authority shall be subject to ___s submission of proof of compliance with the
condition of this authority.
9. Any misrepresentation in the documents and/or information submitted in relation to
LLIs request for herein approval, which may later be discovered by PEZA and/or
non-compliance with any terms and conditions under this Authority and any existing
PEZA, BOC and BIR rules and regulations shall be cause for imposition upon ___ of
applicable penalties under the PEZA Rules.
On the basis of the foregoing, please be informed that ____ is hereby issued a
TEMPORARY PERMIT to proceed with the importation/procurement of machinery,
equipment and other assets and construction/fit-out of its proposed expansion site at
the __________________, subject to the following terms and conditions:

Not later than 30 days from date of this LOA, ____ shall
submit to PEZA its application for registration of its new/expansion project at the
________________.

PEZA Official Receipt for paymet of processing fee of this LOA amounting to
Php 1,200.00;
of its list of importables (goods/items to be imported that are entitled to tax and
duty exemption, for uploading in the PEZA E-IPS.
8. The subject building facility to be leased by _____ shall be used exclusively by
_____ for its PEZA-registered operations;
9. This authority shall be valid for three (3) months on the date of this approval;
10. Any misrepresentation in the documents and/or information submitted by ___ in
relation to its application for herein approval which may later be discovered by PEZA
and documents submitted as required above, and/or non-compliance with any of the
terms and conditions of this authority shall be cause for imposition upon ___ of
applicable penalties under the PEZA Rules, including the suspension or revocation
of this authority.
The PEZA _____ Zone Administrator/Manager/Officer-In-Charge shall be in charge of
implementing this authority.
On the basis of the foregoing, please be informed that ____ is hereby issued a
TEMPORARY PERMIT to proceed with the importation/procurement of machinery,
equipment and other assets and construction/fit-out of its proposed expansion site at
the __________________, subject to the following terms and conditions:

existing PEZA, BOC and BIR rules and regulations shall be cause for imposition
upon ___ of applicable penalties under the PEZA Rules.
The PEZA _____ Zone Administrator/Manager/Officer-In-Charge shall be in charge of
implementing this authority.

5. All withdrawals from the zone of goods to be sold in the domestic market shall be
subject to prior inspection by PEZA and Customs Examiners and shall be covered
by a properly approved PEZA Form 8106. Prior to approval of the first PEZA Form

Administrator/Manager/Officer-In-Charge:
d) an undertaking signed by the highest responsible official of ___ stating that:
(i) ____ shall submit to PEZA, an inventory as may be needed; (ii) ___
understands and accepts that failure to account for the subject importations
shall be sufficient cause for PEZA to withhold issuance of any subsequent
authority to dispose of unused and defective goods, or as may be warranted,
be subject to applicable penalties under the PEZA Rules and Regulations; (iii)
in case bidding was not witnessed by the PEZA _____ Zone
Administrator/Manager/Officer-In-Charge, a certificate from two (2) of ____
highest responsible officials stating that ____ has conducted a bidding for the
sale of the subject goods and the following bids were received by ____

e) in case bidding was not witnessed by the PEZA _____ Zone
Administrator/Manager/Officer-In-Charge, copy of bids received by ____ from
the bidders identified in condition 6(a)iii of this LOA;
6. All sales shall be subject to existing Customs and Internal Revenue rules and
regulations, including payment of corresponding duties and taxes and
internal revenue taxes, which shall be paid prior to withdrawal of the goods
from the zone;
7. Any destruction of the goods at _____s ____-SEZ facility shall be witnessed by
PEZA and BOC Examiners. The withdrawal of the subject destroyed items from the
zone shall be covered by a properly approved PEZA Form 8106;
If the subject destroyed items can no longer be used for its intended purpose, the
PEZA processing fee for scrap sale of Php500.00 per transaction shall be paid;
However, if the subject items will no longer be subjected to physical destruction
and thus are still found to be usable for its intended purpose, the sales shall be
subject to the payment of a PEZA processing fee equivalent to five percent (5%)
based on the actual selling price, based on bidding conducted by ____ and
presentation to the PEZA _____ Zone Administrator/Manager/Officer-In-Charge of
original copy of bids received by ____ for the sale of the subject goods, or the
assessed value from the Bureau of Customs, whichever is higher, exclusive of
customs duties and taxes paid, payment of which shall be made prior to withdrawal
of the items from the zone; and
The Official Receipt (O.R.) No., amount paid, date of O.R.s issued by the Bureau
of Customs (BOC) and PEZA shall be indicated on the PEZA Permit Form 8106
prior to withdrawal of subject goods from the zone;
Miscellaneous and Reportorial Requirements
8. Income realized by ___ under this authority shall be considered/reflected as
Other Income in ____s book of accounts, Income Tax Returns and Audited
Financial Statements, and shall be subject to payment of the Regular Corporate
Income Tax. For this purpose, ____ shall set up separate account for the
revenues/income from its activity, to segregate it from income earned from its
PEZA-registered operations and shall further be reported as a separate item in its
monthly and annual reports on operations submitted to the PEZA-Enterprise
Regulations Department in accordance with Rule XXI, Section 4 of the PEZA
Rules;
9. The sales of ____ under this authority shall be included in the Annual Report on
operations of ____ required to be submitted to PEZA Enterprise Regulations
Department under Rule XXI, Section 4 of the PEZA Rules and Regulations. The
report
shall
be
duly
attested
by
the
PEZA
_____
Zone
Administrator/Manager/Officer-In-Charge and shall indicate the PEZA Form 8106
permit no./date, description, volume and value of items sold, name of buyer, ___
invoice no., Official Receipt No./Date for BOC of payment of customs duties and
taxes, internal revenue taxes, with the amount paid, and O.R. No./date issued by
PEZA for processing fees and amount paid.
10. This authority shall be valid for three (3) months on the date of this approval. Any
extension
hereof
shall
be
issued
by
the
PEZA
____
Zone
Administrator/Manager/Officer-In-Charge, subject to the same terms and
conditions of this LOA;
11. Any misrepresentation in the documents and/or information submitted in relation to
its application for herein approval which may later be discovered by PEZA, and/or
non-compliance with any of the terms and conditions of this authority and any
existing PEZA, BOC and BIR rules and regulations shall be cause for imposition
upon ___ of applicable penalties under the PEZA Rules.
The PEZA _____ Zone Administrator/Manager/Officer-In-Charge shall be in charge of
implementing this authority.
Copy of the certification from PEZA-Enterprise Services Division (ESD) that ___
is up-to-date in its submission of reportorial requirements including the Ecozone
Monthly Performance Report (EZMPR);

Payment of customs duties, fees and charges and applicable revenue taxes,
donors tax as provided in the National Internal Revenue Code (NIRC), unless
exempt therefrom as evidenced by a Certificate of Exemption for duties and
taxes, and/or Certificate of Eligibility of donee as tax-exempt from the
Department of Finance. Said exemption shall be indicated and acknowledged by
the ____-SEZ BOC personnel in the Form 8106.

Movement of Goods
16.
The withdrawal of the subject goods from _____s facility at ____-SEZ shall be subject
to inspection by PEZA and Customs Examiners prior to withdrawal from the zone and
shall be covered by a properly approved PEZA Permit Form 8106 issued by the PEZA
____ Zone Administrator/Manager/Officer-In-Charge.
17.
Within fifteen (15) days from date of withdrawal of the goods from ____s facility at the
LT-SEZ, ____ shall submit to the PEZA _____ Zone Administrator/Manager/Officer-InCharge, copy furnished PEZA-Enterprise Regulations Department (PEZA-ERD) the
following:
a) Notarized Deed of Donation between ____ and ___ (donee)____; and
b) Liquidation report on the donation of the subject goods under this authority,
duly signed by ___s highest responsible official, indicating the following:
PEZA Form 8106 Permit No., date, description, quantity, and original
receipt nos., and date of all fees paid, i.e., customs duties and taxes,
donors tax (when applicable), fees, and charges. The liquidation report
shall also include pictures of recipient/ representative receiving the goods
from the donors.
12. This authority shall be valid for a period of three (3) months effective on the date
of this approval. Any extension hereof shall be issued by the PEZA ____ Zone
Administrator/Manager/Officer-In-Charge, subject to the same terms and
conditions of this LOA;
13. Any misrepresentation in the documents and/or information submitted by TPC in
relation to its request for herein authority which may later be discovered by PEZA,
and/or non-compliance with any of the terms and conditions of this authority shall
be cause for imposition upon TPC of applicable penalties under the PEZA Rules
including the revocation of this authority.
The PEZA _____ Zone Administrator/Manager/Officer-In-Charge shall be in charge of
implementing this authority.
This authority shall be limited to _____s local sale of the total value of
its ____ services, i.e., to engage in _______________ Services undertaken
at its __________________, which shall not exceed the equivalent of ___
percent (___%) of its actual total sales value for CY/FY ____.
For the purpose of compliance with the ___% local sale limit, the said local sales
value of ____s lcal subcontracting services shall be computed to idndicate the
value of services rendered, plus, the value of the goods provided by ____s local
clients and the value of the goods (imported / locally purchased) and other
production inputs provided by ____. For this purpose ___ shall comply with the
ff:
a. Prior to or upon entry of raw materials from ____s local clients to ____s
facility, the application of ____ for PEZA Permit Form 8105 shall include: (i)
the job order/contract documentation/invoice for every client/buyer with
volume and corresponding value of the raw materials provided by the said
local client, copies of which shall likewise be attached as supporting
documents to a monthly summary report to be submitted to the PEZA ____
Zone Administrator/Manager/Officer-In-Charge;
The PEZA Permit Form 8105 shall only be issued by the PEZA ____ Zone
Administrator/Manager/Officer-In-Charge upon ____s compliance with the
above-mentioned requirements;
Prior to the exit of the goods from the zone, ____ shall file its application for
Peza Permit Form 8106 together with the invoice of ____ to its local clients,
showing separately the volume and value of goods provided by the local
clients, and the value of charges of ___ to its local client, which shall include
processing fee plus the alue of additional supplies or raw materials frovided
by ____, if any;
_____ shall maintain a ledger of goods brought into the zone for
subcontracting and processed goods returned to the local client in
accordance with the attached format (Annex A.2 and B.2) which shall be duly
accomplished every time goods are brought into and out of the zone;
b. payment of duties and VAT for the services rendered by ____ and the goods
(imported/locall purchased) provided by ____ and their local clients;
For this purpose, the PEZA Form 8106 should indicate the following
respectively; (i) value of services rendered; (ii) value of imported goods
provided by ____; (iii) value of locally purchased goods provided by ____;
and (iv) value of goods provided by ____ s local clients; and
c. payment to PEZA of a local sale processing fee equivalent to two
percent (2%) of the selling price, but not more than 10,000.00 per
transaction. The PEZA ____ Zone Administrator/Manager/Officer-In-Charge
shall issue an Order of Payment to this effect, based on the monthly
summary report of local transactions.
The Official Receipts (O.R.) Nos., amount paid, and date of the O.R.s issued by
the Bureau of Internal Revenue and PEZA shall be indicated in the liquidation
report.
2.
Not later than _________, _____ shall submit to the PEZA ____ Zone
Administrator/Manager/Officer-In-Charge a clearance from the PEZA Incentives
Management Division (PEZA-IMD) stating that ____ has submitted proof of
remittance of VAT to BIR on their local sales in ___(specify year/s)____.
Failure by _____ to provide the PEZA ____ Zone Administrator/Manager/OfficerIn-Charge with the said clearance from PEZA-IMD, shall be cause for the PEZA
____ Zone Administrator/Manager/Officer-In-Charge to withhold any
authority/permits to ______.

Request for renewal of this authority shall be filed with PEZA on or before
01 December _____, together with the complete certified report on local sales
made under this authority in accordance with the report format attached hereto
as Annex A, covering the period from 01 January _____ up to 15 November
____. In addition, _____I must be up-to-date in its submission to PEZA of
complete reportorial requirements in accordance with Section 4, Rule XXI, of the
PEZA Rules and Regulations. Requests for renewal not complying with complete
requirements shall not be considered filed.
The final report on _____s local sales, to include sales for the period 16
November to 31 December ____ shall be submitted to PEZA-ERD in the
attached format (Annex B) not later than 31 January _____.
It is noted that in accordance with Section 8, Rule XXV of the PEZA Rules and
Regulations, it provides that a schedule of fines and/or administrative sanctions
who have committed violations of said Rules and pertinent circulars/memoranda
issued thereunder: for late filing of financial statements, income tax returns,
performance reports, annual reports and all other reports/documents as
specifically enumerated under Section 4, Rule XXI or required under existing
circulars of the PEZA, basic fine of Php500.00 and daily fine of Php50.00 for the
first violation, basic fine of Php1,000.00 and daily fine of Php100.00 for the
second violation and basic fine of Php2,000.00 and daily fine of Php200.00 for
the third violation.
