I, Jason T. Dennett, declare as follows:


1.

I am over the age of 18 and am competent to be a witness in this lawsuit. I am

counsel for Defendant Windermere Real Estate/Center-Isle, Inc. ("Windermere") and make the
following statements based upon my personal knowledge of the matters testified to herein.
2.

Attached hereto as Exhibit 1 is a true and correct copy of the 2001 Form 22W

that Windermere provided to Plaintiff Jonathan Deegan, and to which Mr. Deegan refers in
Paragraph 9 of his Complaint filed November 18, 2014.
3.

Attached hereto as Exhibit 2 is a true and correct copy of the 2001 Form 22W

that Windermere provided to Plaintiff Alice O'Grady, and to which Ms. O'Grady refers in
Paragraph 10 of her Complaint filed November 18, 2014.
