: Spot Report on Murder Case at B. Mendoza St. City of San

Fernando Pampanga

Reference: Verbal instruction from Ebanada

2. Please be informed that on or about 5:00 oclock in the afternoon of January 13,
2016, a Murder incident transpired at B. Mendoza St. City of San Fernando Pampanga,
wherein the victim was identified as Jan Marco Dizon y Bongalon, 20 years old, resident of

Fernandino St. San Nicolas City of San Fernando Pampanga.

3. Initial investigation disclosed that while the victim was having a drinking spree with
four(4) others at the Convention located at the above-mentioned place. Moments later victim
left the group and proceeded towards a nearby restroom. At this moment, one of the group
identified as Jose Marie Madla 20 years old and resident of same place also left pretending to
call someone from his cell phone but secretly followed the victim at the restroom.
4. At this instance, the suspect suddenly grabbed the victims neck and strangled him
causing his death. To hide the crime, suspect got a rope, tied it to the victims neck and hung
him to show the sad victim committed suicide.
5.
Upon discovery Michaela Rose Orduna reported the incident to the police who
conducted their own investigation. During the investigation, suspect admitted having killed the
victim because of personal grudge.

This pertains to the killing of Jan Marco Dizon y Bongalon on January 13, 2016 at
the Convention, B. Mendoza St., City of San Fernando, Pampanga by Jose Marie
Madla.

3. After the investigation, the police investigators filed a complaint of murder against
the suspect before the Office of the City Prosecutor docketed under NPS No. 132016. Suspect is presently detained at the Pampanga Provincial Jail after the
Inquest proceedings.

Rosario, Purok 1, #94 City of San Fernando, Pampanga.

others at the Convention, B. Mendoza St., City of San Fernando, Pampanga. A few Moments
later victim left the group and go in a nearby restroom. At this moment, one of the group
identified as Jose Marie Madla 20 years old and resident of same place also left pretending to
call someone from his cell phone but secretly followed the victim at the restroom.
4. At this instance, the suspect choked the victims neck and strangled him causing his
death. To hide the crime, suspect got a rope, tied it to the victims neck and hung him to show

Felipe, Zambales for autopsy.

Please be informed the case has been filed in inquest proceedings against the
arrested suspects before the Office of Assistant City Prosecutor, Jake B. Simon of Angeles City
Prosecutor Office on January 16 2016 and docketed under NPS/IS NO. 13-2016. The
Investigating Prosecutor hereby recommended the further detention of the respondents after
finding probable cause on the referred case for Murder.
