Module Description:
This module will introduce students to primary health care
from a philosophical basis and a model of service provision
using the World Health Organisations global framework for
health.

Students will be able to apply health education and health


promotion models as strategies for achieving sustainable
health outcomes.
This module provides students with understanding and
knowledge of primary health care and community
development concepts in multiple healthcare contexts.

Learning outcomes:
Explain the principles and key concepts of primary health
care;
Identify current and future issues in community health
(e.g. communicable diseases, immunisation patterns,
environmental health, alternative health practices,
disability, gender health, domestic violence, child abuse);
Discuss and apply the various concepts related to
population health, wellness, community health services,
health promotion, risk reduction, early intervention and
disease management;
Discuss ways health promotion priorities are defined in a
community;

Learning Outcomes
Apply knowledge of learning styles and learning theories to
health education and health promotion;

Use a health education framework to assess, plan,


implement and evaluate health promotion in the
community;

Discuss the role of healthcare providers in primary


healthcare;

Identify resources available for the promotion of health


and wellness across the lifespan; and

Recognise social, economic and political determinants


of health.

3 fieldwork projects

This series of fieldwork projects is designed to be undertaken


throughout the semester in order to provide students with a range
of community-based learning opportunities which will enhance
understanding of the key concepts and practices of primary health
care.

FIELDWORK PROJECT 1 Weeks 2, 3 4, 5 & 6


Community Health Assessment

In this fieldwork activity you are required to discuss and present a community
profile of the geographic community you have chosen to assess.

This group
presentation will
contribute to 30% of
your total marks

FIELDWORK PROJECT 2 Weeks 5, 7, 8 & 9


Health Promotion in the Singapore Context
(Primary Level of Prevention)

In this project you are required to identify a physical/mental health issue is


covered in your Project 1 community and then name a target group affected
by the health issue. Examine prevention strategies and activities
implemented by the HPB and other community groups to improve the
communitys health at the Primary Level of care.

FIELDWORK PROJECT 3 Weeks 10, 11 & 12


Health Education Intervention
(Secondary Level of Prevention)

In this project you are to design an educational program to address the


educational needs of your population suffering from the chronic disease
phase of the health issue identified in project 2.
