ADAMOS INTERNET CAF INTERNET ACCESS SERVICE

AGREEMENT
Service Provision Conditions
1. Equipment and Peripherals. Adamos Internet Caf shall provide the
Subscriber with the Customer Premises Equipment (CPE) or Router and
other peripherals necessary for Service access as set forth in the
delivery receipt issued to the Subscriber.
2. Service Commencement Date. The Service Commencement Date for
the Service provided shall be the day when the agreement is signed.
3. Service Fee and Other Charges. Upon signing of the subscription, the
Subscriber shall pay Adamos Internet Caf a Monthly Service Fee of
two thousand pesos (Php2000.00) and the processing fee prevailing at
the time of approval of application for subscription, which processing
fee covers the installation fee. In the event or pre-termination by the
subscriber due to non-payment of dues stated herein or otherwise, the
latter shall be charge of the following fees and penalties.
Payment of outstanding balance plus the subscription fee for the
unexpired period in the agreement.
4. Price are based on a minimum contract terms of one (1) year. Contract
term shall be automatically renewed for the same period unless the
customer sends a written notice terminating the service at least (30)
days prior to expiration of the of the initial contract term or renewed
contract term, as the case may be.
5. Subscriber also understands that should he/she fail to settle his/her
account after 60 days or violates the terms and conditions of this
agreement or the provision of R.A. 8484 known as the Access Device
Regulation Act. Adamos Internet Caf shall have the right to
temporary/permanently disconnect internet service even without prior
notice.
6. In the event of CPE and Router malfunction, Adamos Internet Caf will
replace CPE (Customer Premises Equipment) and Router within the
contract period.
7. If the Service provided to the Subscriber is of unacceptable quality (the
quality of the Service to be determined by Speedtest.net by Ookla or
50% below the subscriber availed plan) the Subscriber shall be
entitled to terminate this Agreement and receive a full refund of the
Subscription paid in the previous 30 days provided that the Service is
cancelled and the Equipment is returned to Adamos Internet Caf.
8. Adamos Internet Caf will notify SSC-Bulan of any significant change(s)
in this Agreement. Any such changes shall become effective
immediately except where applicable law requires a notice period, in
which case the change will become effective at the end of the requisite
notice period. Upon effectiveness of any change to any of these
documents, the continued use of the Services will constitute the
consent to such change and the agreement to be bound by the terms
of the document as so changed. If SSC-Bulan do not agree to any such
change, it will immediately stop using the internet service and notify
Adamos Broadband that SSC-Bulan is terminating the internet services.
