Camus Bldg. EDSA Bagong Lipunan, Crame Quezon
City where the initial clientele of Security guards and PNP
Applicants are served.
Morinaga Clinical Laboratory has expanded its services
to serve all possible clients located in the entire country.

MEMORANDUM OF AGREEMENT
Know All Men by these Present:
This agreement entered into by and between:
MORINAGA CLINICAL LABORATORIES, with office address
at Unit 503-f 2nd flr. Camus Bldg. EDSA, Bagong Lipunan,
Crame Quezon City hereinafter referred to as the first party
represented herein by its President Miss. Precy R. Morinaga

WITNESSETH:

The first party provides medical and diagnostic services

including Hematology, Clinical Microscopy, Blood Chemistry,
Immunology/ Tumor Markers Bacteriology, Histopathology,
Serology and other specialized tests.
The second party is in need of the services of the first party
for the purpose of pre employment or Annual physical
examinations or any events that needs Medical Services. Both
parties have agreed on the following terms and conditions:
1. Morinaga Clinical Laboratories shall enter into a contract
with
for the physical examination of employees
which shall remain in full force and effect for a period of one
(1) year. The agreement is automatically renewed for the
same period from year to year, unless terminated upon
written notice given by one party to the other thirty days or
sooner prior to the date of the termination.

Rates of services is attached as Annex A. Mode of payment

preferably cash basis, However terms of payment can also
be every 10th and 25th or 15th and 30th of the month. Delayed
payments will be subject to 10 % penalty charge.
Official Results will be release after a day for 100 and bellow
number of clients/ patients, and two days after for clients
over 100 and above.

4. We follow confidentiality with regards to patients / clients

results with or without findings.
5. For patients/ patients with abnormal findings we have
doctors who can advice, who will manage their case with
confidentiality.
