It has been determined that Guidance and Counseling is an integral part of the
educational process in secondary schools. The unit to be effective must take a team
approach and work with students, parents, teachers, school administrators, and
representatives of community agencies to ensure that the programs and services
being offered are effective and responsive to the needs of its clientele. We must
specifically determine what it is that Counselors should do to answer students'
concerns and it is through needs' assessment instruments that we can best set up
our programs to answer perceived needs of students.
In connection, the office aims to mold a Developmental Guidance Program
intended for the holistic growth and milestone of our beloved students in the
Secondary Laboratory School. For such desire to be achieved, the undersigned is
requesting to allow the conduct of a NEEDS ANALYSIS SURVEY on September
10-11, 2015 at SLS Audio-Visual Room to gather research information on the
prevailing educational, occupational, personal-social needs and presently prevailing
concerns, issues and life battles among adolescents today. These developmental
concerns has significant impact to students learning performance, school
involvement, and personality functioning which should be given appropriate
remedies and intervention.
The office is anticipating for your favorable response and action regarding the
request. Attached is the Schedule of the NEEDS ANALYSIS SURVEY and the
corresponding list of student names, who are randomly selected to be a
part of the sample population, in the SLS Department.

The office would like to humbly request that a GUIDANCE ROOM from the SLS Department be
allocated as a starting point in establishing the important role of Guidance and Counseling in
directing the life of students. Putting up a Guidance Room situated within the department and
solely for secondary students is an enormous step in engaging rapport, building helping
relationship and establishing close monitoring to the developmental milestone, adjustments and
challenges encountered by young ones nowadays.
Furthermore, the implementation of the Senior High School under the K-12 Program is crucial to
understanding factors affecting career decision-making and choice of major or specialization as
a student climbs into a transition from Junior High to Senior High School. Leading the way, we
envision commencing a CAREER GUIDANCE PROGRAM advocating critical directional
guidance to students in the choice of their future career suitable to their aptitude, inclinations
and demand of the job/labor market. Each Guidance Counselor will be on duty for a structured
number of hours each week to revive and extend invitational learning environment and dealing
with presently prevailing issues among adolescents such bullying, pre-marital sex, teenage
pregnancy, abortion, corporal punishment, job-skills mismatch (career choices) and other
concerns.
The office is anticipating for your favorable response and action regarding the request. Thank

Upon the pursuance of the school year, series of activities will be organized by the office in
order to deliver the programs and services intended for the students well-being, self-growth,
and decision-making and life skills. Among the activities prioritized for the semester are the
Needs Assessment Survey for High School and Tertiary level, Orientation to High School and
College Life, Intake Interviews, Career/occupational counseling, Celebration of Career
Guidance Week. Group counseling session, group dynamics, short self-enhancement courses
or PEP classes are among the mechanisms that will be used by the office towards students
satisfactory experience and appreciation Guidance and Counseling Services.
Furthermore, the Class Schedule by Section with adviser and the SLS number of enrollees from
Grade 7-10, forwarded by your good office, is truly a valuable information and aid towards a
smooth planning and conduct of the above-mentioned events. We are grateful for your
immediate action to our request.
In view thereof, we are as well writing for the intent of an additional request for the STUDENTS
LIST (NAMES) BY SECTION FROM GRADE 7-GRADE 10 which is crucial in identifying &
drawing a sample population to participate for the NEEDS ASSESSMENT SURVEY to be
conducted by the office as a basis for developing a DEVELOPMENTAL GUIDANCE
PROGRAM for secondary students.
We humbly hope to receive once again a positive response and action concerning the request.
