Canada - Nova Scotia Offshore Petroleum Board (CNSOPB) digital Data Management Centre (DMC).

This form, once completed, must be signed and faxed to the attention of DMC Support at (902) 468-4584. Once submitted, it will be reviewed by
DMC staff and, if approved, an internet access account will be created and a user name and password will be provided. Subject to certain conditions,
this account may be used to access non-confidential digital well and geological data and exclusive seismic data. Please note the DMC internet
access account is not transferrable.
The CNSOPB does not warrant the information contained in the CNSOPB DMC in any way and therefore is not responsible for any errors or
omissions.
The undersigned (the DMC User) agrees to comply with the terms of the Exclusive Seismic Data - Data Access & Confidentiality Agreement (the
Agreement) as it pertains to the disclosure of digital exclusive seismic data. Before a DMC User is permitted access to exclusive seismic data they
will be required to agree to the terms of the Agreement by clicking the Accept button when the Agreement is displayed. The CNSOPB will monitor
the data accessed by each DMC User. This information will be made available to the data owners and the DMC User hereby consents to the
disclosure of this information.
The DMC User agrees to indemnify and save harmless the CNSOPB and the data owner and their respective affiliates, directors, officers, employees
and agents from and against all actions, claims, damages, losses, expenses and costs, inclusive of special, indirect or consequential damages,
including loss of profit or loss of competitive position by the data owner or DMC User, which they or any of them may sustain, incur or become liable
for as a result of the disclosure of any DMC data to the DMC User or any of the DMC Users affiliates, directors, officers, employees or agents, and
this indemnity shall continue in full force and effect and be applicable regardless of whether any such persons who have caused or contributed to
such breach cease to be employed or engaged by the DMC User. Neither the data owner nor the CNSOPB shall be liable in an action initiated by the
DMC User for special, indirect or consequential damages resulting from the disclosure or use of any DMC data, including, without limitation, loss of
profit or business interruptions, however same may be caused.
The CNSOPB reserves the right to restrict, suspend or terminate any DMC User account, without prior notice.
