DIESEL LOCOMOTIVE DEFECT CODIFICATION FOR

MANAGEMENT INFORMATION SYSTEM
1.

Codification System.

This Codification system has been developed to facilitate collection of information from
Railways about defects of diesel locomotives. This information will be used to identify deficiencies
in design, manufacturing and maintenance system of the locomotives. For this purpose, defects can
be defined as all such occurrences, which affect or could have affected reliability of the locomotives.
All defects whether causing statistical/non statistical engine failure or noticed in shed should be
included.
2. Railways should advise details of the defects as per the following parameters:
Parameter 1
Parameter 2
Parameter 3
Parameter 4
Parameter 5
Parameter 6
Parameter 7
Parameter 8
Parameter 9
Parameter 10

3. Details of defect/failure cases (Parameter 1-5)

No codification for the information from Parameters 1 - 5 is prescribed as this information can be
given in natural style. Information for Parameters 6 - 8 should be as per the system explained
below:CODE

CATEGORY

Y/N

Design changes are needed (Yes/No)

Y/N

Manufacturing methods need (Yes/No)

Y/N

Maintenance system needs improvement (Yes/No)

4. Location and Nature of Defect (Parameter 9)

This parameter contains the information regarding location and nature of defect. For this purpose
a 4- alphabet code has been prescribed as per details given in the Appendix. Information contained
in each of these 4-alphabets is as follows :
1s t alphabet: Indicates the main assembly involved in the failure/defect
(Page 1 of the Appendix)
2nd alphabet: Indicates the sub-assembly of main assembly involved in the failure/defect
(Page 2- 5 of the Appendix)
3rd alphabet: Indicates the sub-subassembly/component of sub- assembly involved in the
failure/defect (Page 6-22 of the Appendix);
4th alphabet: Indicates the nature of defect on the component/sub-assembly/component
involved in the failure/defect (Page 23-119 of the Appendix).
In the event of none of the components involving in failure or defect, the 4-alphabet code of
this parameter will be given as ZZZZ.
Occurrence of 'Z at the 2nd or 3rd or 4th alphabet place indicates that the involved location or
nature of defect is other than the ones listed in the Appendix.
5. Remarks (Parameter 10)
Parameter 10 is the Column for 'Remarks' in which Railways should indicate in brief what
improvement is recommended to prevent occurrence of such defects.
6.

Sample reporting form

A sample Reporting Format is placed at Annexure. A number of illustrations of defects are also
indicated.
