the PCDC MCC Substation 258 Breaker 2 was
used for isolation which shutdown B-6 and
the site network switch located in B-6. The
network switch is a communication hub for
the site.,

noticed a downed power line on Power

The UPS in NaCN, HCN, and Oxone control


rooms alarmed during a power blip. The
NaCN process lost both #1 and #2 systems
shortly after the blip. There were no
operating issues in the HCN area. Oxone was
already down.

KSW Chemical Room Lost Heat to Area. HVAC


Crew responded. A hot terminal was found on
the local disconnect to unit 3092

this call the EU supervisor received an e-mail


indicating an UPS alarm. The EU
supervisor investigated and found the UPS
system had switched to un-conditioned
power and the Building #11 fire alarm panel
had a trouble alarm. The EU supervisor
contacted the Electrical supervisor. The
Electrical supervisor was also called
regarding the shutdown of the smoke duct
scrubber system. This system shut down at
7:45am. This caused the CP operators to
reduce shop power at 7:57am. Power was
reduced to ~33,500 amps and again at
8:42am to ~27,500 amps., The EU and
Electrical supervisor investigated the area.
The DCS system for the smoked duct
scrubber was operational but the CP
operators could not operate the pumps or
blowers. After further investigation
instrument panel IPP-11 had failed. This
panel provides power to all the smoked duct
scrubber instruments and DCS power for the
pumps and blowers. This panel also powers
Building #11 elevator fire alarm panel. Panel
IPP-11 is powered from the UPS system. The
panel failed due to water infiltration, site
conditions and panel integrity. The failure of
this panel caused the UPS system to switch
to un-conditioned power, as designed. Panel
IPP-11 is scheduled to be replaced with the
existing capital project to replace/upgrade
the smoked duct scrubber project. , The e

Friday December 13th, Operations was


attempting to restart the Vortac Mill after a
shutdown. E&I was contacted to help and
they began troubleshooting. , 4pm - E&I
mechanic entered ECR and found 440V
starter inoperable.

Blower motor rpms dropped from normal to


about 80%. The blower was running by VFD
#2. A burning smell was noticed in DHN ECR,
particularly from the switch in the middle of
the ECR that connects either blower to the
motor. The blower motor then tripped. A high
motor winding temperature fault was
observed on the VFD and SEL Motor
Temperature Relay. With the motor down, the
knife blade switch for VFD #2 was opened
and closed a few times. Through a plexiglass
window on the switch it was observed the
switch was not making full contact when in
the closed position. The bad connection
caused the smell in the ECR as well as the
motor high temperature trip. The
temperature fault was reset on the SEL relay,
and VFD #1 selected and the blower
restarted. The VFD #2 switch will have to be
repaired when power is of the motor before
VFD #2 can be selected to run the blower.

An IR scan was conducted on B dryer oil


heater panel. A category 3 hot spot was
found at the oil heater contactor.

An overhead cable was found laying on top of


the NaCN Maintenance Shop roof on the
north and west sides. Noted that this is one
of four overhead cables which were found to
have been pulled loose from a single
connection point located on a wood pole
across the northside drive from the NaCN
Maintenance Shop. A trash container was
found located/staged directly under the cable
which is routed over the road and above the
NaCN Maintenance shop.

A light fixture short circuit next to the #4 EQ


tank. It appears that the light, which is
attached to grating underneath the tank, was

A contract electrician was using a boom lift to


work on a failed light in the west plant curd
area over the sharple bay. The light was a
277 volt GE type H2000 with a plug in type
removable light fixture. An arc occurred in
the process of removing a screw from the
retaining plate on the light fixture. The
electrician received a first degree burn from
the arc flash on his right wrist. He was
wearing leather gloves and long sleeves. The
process of bending his wrist exposed a small
area of the wrist which received the burn.
The circuit the light was on did trip the
breaker.

I/E Maintenance received a Work Order


(91331758) to repair lights in the cooling
tower area. I/E Technician stopped at the
lighting panel to inspect the breakers. I/E
Technician observed that there were two
(20amp) breakers that were tripped in the
lighting panel. I/E reset one of the breakers
which caused a trip of the Main Feed Breaker
for MCC-1. The trip of MB-1 was not
anticipated because of two other (50amp)
breakers in series with this circuit to protect
the MB-1. The trip resulted in the loss of
power to the Aniline Unit distillation pumps,
Administration Building and Control Room
Building lighting.

The UPS in NaCN alarmed in the Control


Room and the Caustic Unloading Pumps
shutdown. The UPS alarmed in the Oxone(R)
Control Room, but no operating equipment
was afected. HCN and Power did not
experience any issues or alarms.

in the 705 trailer was discovered to be of.
The HVAC breaker had tripped due to internal
breaker failure. There were no injuries

0805 the Lockout relay located in Substation
45 tripped the Breaker # F-22 located in
Switch Center 4 causing loss of Power to

Following repairs to the cartridge heater on


the PFA extruder (which required splicing of
the wires to the heater) an electrical arc
occurred from the center die heater to the
metal plate covering the center die cavity.
The arc occurred from the repaired wiring. An
area mechanic was in the process of setting
blades on PFA extruder when the arc
occurred. The mechanic's hands and face
were in close proximity to where the arc
occurred but was not shocked.

On the 24th of November at approximately


10:00 AM line 1 tripped due to a flame
scanner failure on the BMS. Upon further
investigation it was discovered that there was
water intrusion into the proximity sensor on
the amine gas valve on the burner skid. This
water intrusion shorted out the proximity
switch, thus shutting down the BMS as they
are on a common circuit.

Contractor employee unplugged the ice


machine in the Building 6 Break room (due to
loud noise) in preparation for a meeting. As
the plug was pulled from the GFCI protected
outlet, an arc occurred which damaged the
electrical wall outlet and the electrical cord of
the ice machine.

Mississippi Power experienced a fault at one


of their Sub-Stations that resulted in a 30%
Voltage Dip for 3.5 Cycles to the Plant. This
caused the LN 1 & 2 TRI Blowers, LN 2 Ore
Dryer, LN 2 SCD Pumps, LN 2 Liquids Pumps
and Recycle Water Pumps to shut down.

When attempting to regenerate #2 Softener,


the controller would not move into
regeneration mode and the "Utility Failure"
indication light turned on. Upon further
investigation it was found that the "Utility
Failure" light was on for all softeners. While
troubleshooting cause for "Utility Failure"
indication light, it was determined that the
circuit breaker on the 24V DC System had
failed.

Press LWB B4 is the designated conventional


molding cycle press. Press was in the reheating phase when operator of the press
and a nearby maintainance technician
smelled something overheating.

At or around 12:20 am on 11/14/2013, a


partial power outage to the plant occurred.
Upon resetting the drives, the operator
began to notice alarms in the capacitor bank
system. The on-call maintenance technician
was called into the plant. Upon investigation,
#2 Capacitor bank was found inoperable with
indications of a possible arc flash and/or fire.
At that point, the maintenance technician
called in another technician into assist in a
full lockout of the system. Learnings: Multiple
amperage spikes found 30 minutes prior to
the failure. System was completely rewired in
October 2013 No surges seen from electrical
power grid. The fire had extinguished itself
prior to being found.

The curd operator notified the shift lead


operator who then notified the on shift
supervisor, that he was investigating a
strange noise when he found the # 1
sharples defoamer pump running with out
the motor fan guard and the junction box was
tore away from the motor with the wires
exposed. He immediately shut down the

out. There was no one in the area during this
time, and there is no root cause as to what
caused this incident at this time.

hinge and was pinched by the hinge when
the door to the panel was opened.

trouble alarm for #14 & #15 Boiler UPS.
Upon further investigation it was determined
that the transformer failed by a short circuit
from the coil to the case on the harmonic
winding of the transformer. This was caused
by loosening of the transformer steel stack

were consecutively lost, producing low

start-up. Operations attempted to close the
switch on the #1 motor, and proceeded to
field start the motor. Upon field start, the

the main Aggreko High Line Panel located on


the Northeast corner of the EP58 Towers. The
tower was discovered to have no fans
opperating/tripped by the operations staf at
12:38 AM September 3. The PSS was
contacted and was unable to get electrical
support overnight and the tower was run in
its tripped condition until the next morning.
On investigation the morning of September
3rd, the Aggreko Tower Technician and the
URS Electricians found the main breaker for
the West Tower tripped in the High Line
Panel. They started the process of
troubleshooting the cause of the trip ~9:15
AM. Using the barricade suit, the URS
Electrician turned of the West Tower
Disconnect switch on the side of the panel.
He then openned the West Tower Panel and
found no obvious signs of shorting. The #4
motor starter was in the of position having
tripped overnight. The electrician turned of
each of the remaining 9 individual fan motor
starters so that the West Tower would be
denergized when they turned on the High
Line Panel Breaker. The High Line Panel
Breaker for the West Tower was then turned
on. Nothing happened in the West Tower
Panel. The URS Electrician then moved to the
West Tower Panel and turned on the West
Tower Disconnect Switch energizing the
panel. He was then about to begin the
process of energizing the #1 Fan motor st

the unit of and investigated and noticed that
the power cord leading into the conduit box
at the motor had been pulled out from the
bushing exposing the internal insulated
wires. Upon observing this, the operators
then placed a lockout back on the unit's plug
and reported it to management., The
employee who applied the removed lock
returned the call around an hour after the
original attempt and verified the issue with
the wire, which was the reason the unit was
locked out and not underneath the
centrifuge. The employee did relay this
information to the Lead Operator

Cooling Tower Water Supply Valve Closed also
openned the Well Water Supply line to EP58
A-D. This Caused a cross connection between
the Site Well Water system and the EP58
Cooling Tower System allowing well water to
flow in to the lower pressure EP58 cooling
Tower System. The Well water supply line was
a 12" line which quickly caused the EP58
Cooling Tower Coldwell Tank and Tower Basins
to overfill and loose containment. Because of
the tower configuration, water flowing out of
the Cooling Tower basins tripped 8 of the 30
Cooling Tower fans. Two of these fans were
later restored to service, the other 6 fans are
possibly shorted out and will require
replacing. Besides the Cooling Tower Fans,
the overflowing water tripped out the 250 HP
Primary Cooling Tower pump at the Highline
breaker. The system controls worked as
designed and automatically energized the
secondary supply pump maintaining water
flow and cooling to the process. The primary
cooling Tower pump motor and power circuit
were evaluated for condition and found to be
in full working order. The Coldwell tank
overfilled inundating the Tank level
transmitter.

control power for the pump drives in that

upstairs to the main control room to remove

ECR 1. The operator went to ECR1 to
disconnect power source and the power
source was already in of position and locked
out. Process had been in operation up to this
point. Team started the investigation. The
drawings were pulled from the shop file,
where there were two versions, each having
the power source in diferent locations. The
most recent project drawing showed the
power source in ECR 7, MCC 1. Team went to
ECR 7 but could not locate the power source
for the Lamella Feed Pump. The team also
noticed the Lamella Settler Agitator was in
B519, ECR-7, which they recognized as being
in ECR1 above the Lamella Feed Pump. Both
of which were locked out for agitator related
work on 4/26/13. However, the Lamella
Settler power source in B519 was energized
and the agitator was running. E&I checked
starters and determined the Lamella Feed
Pump was located in the unlabeled bottom
compartment. Tags on, both, the field switch
for the Lamella Feed Pump and the Lamella
Settler Agitator refered to a drawing and
power source pertaining to ECR 1. However,
both power sources were located and verified
as being correct in ECR-7. E&I placed new
tags on power source in ECR 7. FLS
commended the operators for taking the
proper steps of LTT. Information tags were
hung on starters in ECR 1 indicating new
location of starter.

A flash was observed on Lucite's substation

of the site wells dropped offline when the

transient event. Entergy is investigating the
event and will follow up with more details as

heaters are all tied together to a single fuse.
This design feature allowed failure to occur
through a single heater wiring without
tripping the fuse. An FCR has been issued to
change this design to individually fused
heaters.

treatment rendered at site medical was
standard protocol for electrical exposures.

When an operator went to field to load a


trailer, he noticed an electrical outlet had
been damaged by the trailer. The reason the
outlet was damaged was because the trailer
tires were changed from double to super
singles when the new trailer series was built.
The super single tires were slightly wider
which caused the bumper to extend slightly
further than it had previously. There is a box
for the spotter to back the trailer into to
prevent hitting process equipment. Because
the new trailer series was slightly wider, it
clipped an outlet that was mounted on the
steel support beam., The new trailer series
was covered under a TA, however the wider
frame was not anticipated to introduce any
new hazards. The area was examined to
ensure that the box at the loading spots
would still prevent the new trailers from
hitting any process equipment. The electrical
outlet was not identified at the time to have
the potential to be hit by the trailer., Due to
this incident, the area was surveyed again to
make sure that there was nothing else that
could be damaged as a result of the wider
frame and everything was found to be in an
appropriate location. No further investigation
required.
