MEMORANDUM OF AGREEMENT

TERMS AND CONDITIONS UNDER WHICH

ON THE JOB TRAINING PROGRAMS ARE CONDUCTED
The

agrees
to
permit
___________________________________ (Student) to have their on-the-job training in this
establishment.
In addition it is agreed that:
1. The company where the training is to be conducted will not be held liable for any accident/ injury
that the student trainee may suffer from training period outside the companys premises.
2. Normally, the student-trainee is not entitled to wages or benefits from the company where the
training is conducted, except when the company deems it necessary to provide for stipend.
3. The student will abide with the companys rules and regulations and will comply with the imposed
guidelines, otherwise, his training program will be cut short and he/she will be excluded from
further participation in the company training programs.
4. The student while in the process of training will have the status of student-trainee neither
displacing a regular worker now employed nor substituting for a worker needed by the company.
5. The student trainee will exercise diligence in any task assigned to him/her.
6. The College shall not be liable for any and all liabilities or any damages to property or injury to
third person, which maybe occasioned by the students intentional, negligible, or irresponsible
acts while in the course of his training.
7. The training program is designed to run for 250 hour as per requirement of their IT 33 practicum
program.
8. The training period begins on _______________ and extends up to ______________.
9. The nature of training will revolve in computer related works such as, but not limited to, IS
Design/Development, Software/Hardware Maintenance, Programming, Technical Support, Office
and Documentation.
10. The adviser shall make supervisory and instructional visits/follow-ups to the establishment during
training period and evaluate the students progress and to discuss training problems.
11. The adviser or coordinator shall have the authority to transfer or withdraw the student trainee
any time.
12. The student has the right to ask for a change of training station. His/her request shall be
evaluated by the school representative and shall be granted if properly justified.
13. All complaints regarding the conduct and performance of the students-trainee shall conform to all
laws & regulations.
The undersigned have thoroughly reviewed the documents and hereby signify this agreement to these
provisions.
