Dear Fname Lname:

You may be eligible to receive a tax remedy benefit increase if your PERS benefits are subject
to Oregon income tax due to Oregon residency.
The Oregon Department of Revenue (DOR) provides PERS with Oregon income tax records
from benefit recipients; however, PERS is unable to determine your residency status using 2014
tax records. DOR may not have reported some tax records that were filed by the extension
deadline and not yet processed.
To receive the tax remedy increase during calendar year 2015, you must certify your residency
status and that your PERS benefits are subject to Oregon income tax as an Oregon resident. This
certification can be made by using PERS Online Member Services (OMS). OMS is available on
the PERS homepage at http://oregon.gov/PERS.

Find the Resources column, and click on Online Member Services (OMS): Log on or
create an account.
If you have an existing OMS account, click on the Log In link, and complete the log-in
process.
If you do not have an OMS account, click on Open a New Account on the Log In page,
and provide the required information. Use your new User ID and Password to complete
the Log In process.
After you log in, select the View/Update Residency link, and complete the three steps to
update and certify your residency.

If you are unable to provide a residency certification using OMS, complete the enclosed form,
and mail it to PERS in the enclosed postage-paid envelope.
You must certify your residency before December 15, 2014, to receive a tax remedy benefit
increase effective with your January 1, 2015 payment.
If you need further assistance, please contact PERS Customer Service at 888-320-7377.
Sincerely,
Benefit Payments Division

Dear Fname Lname:

You currently receive a tax remedy benefit increase because your PERS benefits were subject to Oregon
income tax. To continue to receive this tax remedy increase in the 2015 calendar year, your PERS benefits must
be subject to Oregon income tax because you reside in Oregon. The Oregon Department of Revenue (DOR) may
not have reported some tax records that were filed by the extension deadline and not yet processed.
Legislation passed in 2013 eliminated the tax remedy for benefit recipients whose payments are not subject to
Oregon income tax because the recipient is not an Oregon resident. PERS determines whether a recipient is an
Oregon resident based on income tax records from DOR or residency certifications filed with PERS.
DOR tax records as of November 1, 2014, or your residency certification on file at PERS, indicate that you are
not paying taxes as an Oregon resident at this time. Effective with your January 1, 2015 PERS benefit, the tax
remedy increase will be removed from your monthly benefit.
If your residency status has changed such that you are an Oregon resident for income tax purposes, you can notify
PERS of this change by using PERS Online Member Services (OMS). OMS is available on the PERS homepage
at http://oregon.gov/PERS.

Find the Resources column, and click on Online Member Services (OMS): Log on or create an account.
If you have an existing OMS account, click on the Log In link, and complete the log-in process.
If you do not have an OMS account, click on Open a New Account on the Log In page, and provide the
required information. Use your new User ID and Password to complete the Log In process.
After you log in, select the View/Update Residency link, and complete the three steps to update and
certify your residency.

If you are unable to provide a residency certification using OMS, complete the enclosed form, and mail it to
PERS in the enclosed postage-paid envelope.
You must certify your residency before December 15, 2014, or PERS will remove the tax remedy increase from
your benefit. This removal will continue for all payments in the 2015 calendar year even if you return as an
Oregon resident.
PERS will send a letter with additional details describing the adjustment to your monthly PERS benefit by
December 31, 2014. This letter will include your appeal rights if you disagree.
You can find additional information on the PERS website at http://oregon.gov/PERS.
If you need further assistance, please contact PERS Customer Service at 888-320-7377.
Sincerely,
Benefit Payments Division
