DBT generalmente se muestra superior en efcacia teraputica a otras intervenciones
habituales (Koons et al., 2001; Verheul et al., 2003), y lo que es ms importante,
aporta evidencia emprica respecto a qu tcnicas o intervenciones podran ser ms
efcaces para el tratamiento del TLP.
Las recomendaciones derivadas de los ensayos clnicos sobre la DBT podran
resumirse de la manera siguiente: la terapia dialctico conductual es una terapia indicada
en contextos de atencin comunitaria, es una terapia recomendada para mujeres con
diagnstico TLP, especialmente si presentan tendencias suicidas y auto-lesivas, trastor-
nos crnicos del Eje 1, (de manera especfca los trastornos por abuso de sustancias
que adems son un factor de riesgo asociado a suicidio consumado), conductas de ira,
impulsividad y agresiones, situaciones en las que su combinacin con el psicofrmaco
olanzapina pueden incrementar los efectos teraputicos del tratamiento.
Independientemente de la eficacia ampliamente documentada de la terapia
dialctico-conductual para el tratamiento del TLP, existe otro grupo de investigacio-
nes que presentan evidencias sobre la bondad de otros tratamientos psicolgicos en
el tratamiento de este trastorno mental y del comportamiento. Pueden considerarse
intervenciones probablemente efcaces porque o bien se basan en estudios que todava
no han sido replicados, o bien porque dichas intervenciones no han sido probadas por
investigadores diferentes. Cabe tambin la posibilidad de que el nmero de sujetos
incluidos en los grupos experimentales no supere el mnimo de rigor exigido. Estos
tres criterios son fundamentales de cara a considerar un tratamiento como intervencin
basada en la evidencia (Chambless et al., 1993; Chambless et al., 1998; Chambless y
Ollendick, 2001). En la tabla 2 se muestran los principales hallazgos de estos estudios.
Por ejemplo, Davidson et al. (2006) pusieron a prueba si la terapia cognitivo-
conductual (TCC) junto con el tratamiento habitual (TAU) resultaba ms efcaz que este
ltimo por s solo a la hora de reducir el nmero de hospitalizaciones, asistencias a
urgencias hospitalarias y conductas de tipo auto-ltico en personas con TLP. La TCC se
centr en las creencias disfuncionales y en las conductas que resultaban disfuncionales
en el contexto de cada participante. El anlisis de los resultados mostr que el grupo
que recibi TCC disminuy signifcativamente los intentos de suicidio, si bien no se
observaron diferencias ni en el nmero de ingresos de los participantes ni en la frecuen-
cia en el uso de los dispositivos sanitarios de emergencias (salud mental). Este grupo
present asimismo un menor nmero de sntomas psicopatolgicos y una disminucin
de las creencias disfuncionales caractersticas en el TLP. Adems, trascurrido un ao
de seguimiento desde la fnalizacin de las intervenciones, en el grupo TCC disminuy,
tambin signifcativamente, la variable ansiedad situacional. En nuestra opinin, la TCC
guarda algunas similitudes con la DBT en cuanto a que en ambos enfoques se abordan
conductas disfuncionales. Sin embargo, mientras que bajo la TCC se someten a prueba
esquemas cognitivos y conductas de afrontamiento cuyos resultados son desadaptativos,
en la DBT se hace ms nfasis en el aprendizaje de habilidades especfcas para la reso-
lucin del malestar emocional y de los confictos interpersonales. Pero a pesar de estas
diferencias, quizs la TCC sea ms efcaz que el tratamiento habitual o que otro tipo
de intervenciones por cuanto que trata de abordar determinados objetivos teraputicos
comunes a los establecidos en la DBT.

En base a los tratamientos probablemente efcaces incluidos en este epgrafe
podemos concluir lo siguiente: primero, que la terapia dialctico-conductual cuenta con
mayor apoyo emprico que los diversos tratamientos cuya efcacia no ha sido completa-
mente demostrada (terapia cognitivo-conductual, terapia centrada en esquemas, atencin
plena o mindfulness, terapia basada en el entrenamiento para la resolucin de problemas,
terapia cognitivo-analtica y terapia basada en la transferencia); segundo, que entre los
tratamientos probablemente efcaces, la terapia cognitivo-conductual y la terapia basada
en la transferencia parecen ser mejores opciones de tratamiento en comparacin con el
resto de terapias alternativas; y por ltimo, que determinados tratamientos farmacolgicos
como la olanzapina y la fuoxetina pueden contribuir a la mejora de la efcacia de la
DBT frente a otro tipo de intervenciones.
LIMITACIONES DE LOS ESTUDIOS ANALIZADOS
Los resultados que muestran los distintos estudios incluidos en el presente trabajo,
aunque son cuanto menos esperanzadores, deben ser tenidos en cuenta con cautela, y
ello es debido entre otras cosas a diversas difcultades metodolgicas. Por ejemplo,
una de ellas es el reducido nmero de pacientes de algunos ensayos controlados. En
algunos casos se ha trabajado con menos de 30 pacientes en las distintas condiciones
experimentales. En ensayos como el de Farrel, Shaw y Webber (2009) incluso solo se
han incluido a mujeres, mientras que Chanen et al. (2008) trabajaron con una muestra
de adolescentes que a priori pensamos no representa la entidad nosolgica que aparece
caractersticamente en los adultos. Por tanto, por el momento, es difcil generalizar con
contundencia los resultados de las intervenciones al resto de la poblacin con TLP.
A lo anterior hay que aadir otra difcultad adicional, y es que a la hora de
comparar estudios, aparecen difcultades relacionadas con los criterios diagnsticos
establecidos y los criterios exigidos para defnir la muestra de participantes con TLP,
lo cual es especialmente relevante cuando se piensa en sealar cules son las variables
dependientes y si stas son comparables en los diferentes trabajos de investigacin. En
la misma lnea, surgen algunas difcultades cuando nos preguntamos por los instrumentos
empleados para medir dichas variables dependientes. En general existe una heterogeneidad
importante. Planteamientos similares se pueden realizar con las caractersticas propias de
las diferentes formas de intervencin y procedimientos empleados en las investigaciones.
Otra cuestin importante que hace complejo llegar a conclusiones defnitivas es
que las intervenciones administradas en los estudios se presentan a modo de paquetes
de tratamiento, integrados por ingredientes en parte desconocidos. Esto no permite de-
limitar cules pudieran ser los elementos activos o los elementos comunes a distintos
tratamientos. No obstante, Soler et al. (2009) s han mostrado de alguna manera la
efcacia del entrenamiento en habilidades como componente especfco de la DBT para
el tratamiento de los problemas emocionales y relacionales propios de los pacientes
con diagnstico de TLP.
Otra limitacin que encontramos habitualmente en este tipo de ensayos tiene que
ver con el intervalo de seguimiento de las medidas de resultado empleadas en los estudios.

As, por ejemplo, un seguimiento de 1 ao puede no ser sufciente para comprobar la
efectividad a ms largo plazo de las intervenciones. Es de esperar que cuanto mayor sea
el intervalo de seguimiento, mayor ser la potencia de comprobarse el mantenimiento
de los resultados de las intervenciones psicolgicas para el tratamiento del TLP.
Es conveniente sealar que el trabajo con adolescentes es todava escaso, con lo
que debera considerarse a modo de experiencia piloto. Adems, pensamos que en esta
franja de edad posiblemente no queda debidamente representada la entidad nosolgica
que aparece caractersticamente en los adultos.
Por ltimo, otra limitacin importante tiene que ver con los criterios alternativos
para la validacin de los tratamiento psicolgicos propuestos por Carrascoso y Valdivia
(2007). Casi todas las investigaciones realizadas se han basado en criterios de efcacia
de los tratamientos, en situaciones de difcil replicacin en nuestro contexto clnico, que
es claramente heterogneo y variable, tanto por las diversas peculiaridades del paciente
y de su entorno como del profesional que lo atiende. Introducir estas variables, sin duda
ayudara a generalizar los hallazgos disponibles no solo a nivel de efcacia sino tambin
a nivel de efectividad y de efciencia de las terapias, lo cual es si cabe ms necesario
en nuestro contexto asistencial actual.
CONCLUSIONES
La terapia dialctico-conductual parece ser la primera opcin teraputica para la
atencin comunitaria de las personas con diagnstico de trastorno lmite de la person-
alidad, especialmente si se trata de mujeres con tendencias suicidadas y auto-lesivas,
con trastornos crnicos del Eje 1, con trastornos comrbidos por abuso de sustancias (lo
que constituye un factor de riesgo asociado a suicidio consumado) y con conductas de
ira, impulsividad y agresiones. Probablemente, su combinacin con Olanzapina podra
incrementar los efectos teraputicos del tratamiento.
En cuanto al resto de terapias, aunque con menor grado de evidencia, parece ser
que la terapia basada en la transferencia y la terapia cognitivo-conductual contribuyen
de manera signifcativa a la reduccin de conductas que suponen un riesgo para la vida
de estos pacientes, aunque todava existen datos contradictorios. Y aunque hay datos
que apuntan a la efcacia de la terapia cognitiva y de la terapia interpersonal en el trata-
miento de la depresin mayor en personas con TLP, son necesarios nuevos estudios que
con muestras mayores confrmen la efcacia clnica de estas modalidades teraputicas.
Hay algn dato que sugiere que es ms probable que la terapia centrada en esquemas
cognitivos sea ms efectiva y menos costosa para el tratamiento que la terapia basada
en la transferencia. De hecho, en formato grupal, la terapia centrada en esquemas se
ha mostrado efcaz en la reduccin de sntomas y mejora del funcionamiento global.
Finalmente, el STEPPS aadido a tratamientos habituales parece contribuir signifca-
tivamente en la mejora de la impulsividad y el pobre ajuste social, dos caractersticas
de los pacientes con TLP.
Independientemente del modelo teraputico de trabajo, el abordaje del TLP
requiere un tiempo de tratamiento largo, basado en el establecimiento de una buena

relacin teraputica o rapport con el paciente, puesto que solo as se puede obtener el
compromiso necesario del paciente para continuar avanzando en la terapia. El marco
teraputico debera basarse en el establecimiento de unos lmites claros. E independi-
entemente del origen psicgeno del trastorno (diferente segn el marco psicoteraputico
de trabajo), lo prioritario es disminuir las conductas de riesgo para entrenar despus a
los pacientes en la adquisicin de habilidades (cognitivas y conductuales) que les per-
mitan resolver los confictos del aqu y ahora, abordar los sentimientos de soledad o
abandono y desplegar otras conductas ms funcionales para la aceptacin de s mismos
y para el mantenimiento de las relaciones con los dems. Y para eso, es importante
validar sus propias experiencias y decisiones, como alternativa a lo que siempre le ha
supuesto un ambiente invalidante.
Asimismo, la investigacin tendra que responder a las preguntas de los clnicos
sobre cul es el abordaje recomendable para los mltiples y variados tipos de sntomas
y problemas propios de las personas con TLP. En este sentido, apostamos por estudios
realizados no desde perspectivas categoriales, si no desde una visin dimensional y
funcional del sntoma, y en un contexto ms centrado en la persona.
