HOSPITAL SERGIO BERNALES JEFE SERVICIO DE OBSTETRICIA COORDINADOR COMIT PREVENCION MM ASESOR COMIT PREVENCION MUERTE MATERNA MINSA DOCENTE UNIVERSITARIO vilchezjm@yahoo.es FISIOPATOLOGIA FACTORES DE RIESGO CUADRO CLINICO Superficie placentaria materna: por qu ocurre la hemorragia materna ? la remocin de la placenta deja una superficie de 20 cm. de dimetro que contina sangrando si la musculatura uterina no se contrae y queda contrada FLUJO SANGUINEO POR LA ARTERIA UTERINA Recordemos que hasta antes del parto fluian por ste lecho aproximadamente 600 cc de sangre por minuto EL MIOMETRIO ESTA COMPUESTO DE UNA RED UNICA DE FIBRAS MUSCULARES ENTRELAZADAS LOS VASOS SANGUINEOS QUE ALIMENTAN EL LECHO PLACENTARIO PASAN A TRAVES DE ESTE ENREJADO DE MUSCULO UTERINO (BASKETT 2000) LA CONTRACCION DEL MIOMETRIO ES LA PRINCIPAL FUERZA IMPULSORA, TANTO PARA LA SEPARACION DE LA PLACENTA COMO PARA LA HEMOSTASIA, POR MEDIO DE LA CONTRACCION DE ESTOS VASOS SANGUINEOS vasos sanguneos rodeados por fibras musculares uterinas ESTE MECANISMO QUE PRODUCE UN AHORRO DE SANGRE SE CONOCE COMO SUTURAS FISIOLOGICAS O LIGADURAS VIVAS (BASKETT 2000) (1-10) Las causas mas comunes de HPP incluyen: HPP Atnica: Falla de Contraccin adecuada del tero despus del Parto, la cual explica hasta 90% de la HPP en la mayora de pases; es la ms prevalente y la principal causa de MM en pases de ingresos medios y bajos HPP Traumtica: Trauma al Tracto Genital la cual explica cerca del 7% de la HPP El sangrado debido a Retencin de Tejido Placentario y el de la Falla en el Sistema de la Coagulacin, los cuales explican el restante 3%. 1.
