Accident/Incident Investigation

Policy Statement
[Company Name] requires all employees to immediately report to their supervisor all
accidents and incidents that result in injury or property damage, and all near misses
with the potential for serious injury or property damage !upervisors will report the
accident promptly to management to ensure timely su"mission to #!I$ %ach incident
will "e analy&ed to determine causes and contri"uting factors and the analysis will "e
used to reduce or eliminate the ris' of further incident
Definitions
An Accident is defined as an unplanned event that causes harm to people or damage
to property Accidents are categori&ed as one of the following(
Lost Time Injury )*+I, refers to any injury that prevents a wor'er from coming to
wor' on the day following the day of the injury
Medical Aid refers to any injury not severe enough to warrant more than the day
of injury off, "ut where medical treatment "y a doctor is given
First Aid refers only to injuries that can "e treated on the jo" without any days
lost
An Incident is defined as property damage "ut with no injury to wor'ers
A Near Miss is a situation in which no injury or damage occurred "ut might have
if conditions had "een slightly different
Occupational Illness is defined as a condition resulting from a wor'er-s
e.posure to chemical, "iological or physical agents in the wor'place to the e.tent
that the health of the wor'er is impaired
Critical Injury is defined as an injury of a serious nature that(
a) /laces life in jeopardy0
b) /roduces unconsciousness0
c) 1esults in su"stantial loss of "lood0
d) Involves the fracture of a leg or arm "ut not a finger or toe0
e) Involves the amputation of a leg, arm, hand or foot "ut not a finger or toe0
f) Consists of "urns to a major portion of the "ody0 or
g) Causes the loss of sight to an eye
Role of Superisor in an Accident Inesti!ation
+he !upervisor and the !ite 2ealth and !afety Coordinator if applica"le, must
investigate all accidents and incidents that involve wor'ers +his includes completing
the Accident Investigation 1eport, ta'ing statements from witnesses and collecting any
other pertinent information and ensuring the injured wor'er has received the necessary
medical assistance
+he supervisor is responsi"le for ensuring that all accident reports are transmitted to the
2ealth and !afety 3epartment as descri"ed "elow If a wor'er sustaining a 4irst Aid
later see's medical aid, the supervisor must advise the 2ealth and !afety 3epartment
and have the treating practitioner complete a 4unctional A"ilities 4orm
If we are not the Constructor, report the accident to the Constructor through their !afety
Coordinator or /roject 5anager
+he supervisor should contact the injured wor'er as frequently as the injury deems, or
at least once a wee' If you require assistance, contact the 2ealth and !afety
3epartment
Procedure"
1. +he employee reports a wor' related accident
2. Administer first aid as required
3. Arrange for transportation for injured employee to medical treatment if required
4. %nsure 1eturn to #or' pac'age accompanies wor'er
5. %liminate the ha&ard if possi"le or guard the accident scene if wor'er is critically
injured
6. Investigate the cause of the accident and report findings in the Accident/Incident
1eport form %nsure all areas of the form are completed
7. !end copy of the form to 2ealth and !afety 3epartment
8. 1eport all accidents/incidents as follows(
