Standard 1
6.03-Recognizes and identifies own feelings
Will continue to work on self- awareness while on the floor. Being mindful of the presence of feelings and stopping to interpret these feelings and identify them.
Read the article Making the workplace healthier, one self-aware nurse at a time from http://www.americannursetoday.com /article.aspx?id=9292 Focusing on the importance of being in tuned with myself, taking care of myself and my own feelings and being self-aware.
Will continue to utilize my journal and post-conference to discuss my experience and feelings from the day on the floor.
Will pick a feeling every day that I experienced on the floor and identify it I did focus on self-awareness, and identified when I was feeling anxious, nervous, stressed or overwhelmed. I became mindful of these feelings, and then worked on ways to decrease these feelings (i.e. taking a few deep breaths)
I read the article noted, and really acknowledged the importance of being aware, and mindful of myself and feelings and how the influence my care, my presence, and my well-being.
Have utilized my journal and post-conference to recognize and discuss feelings- such as feeling really overwhelmed @ clinical sometimes, and how this affecting my ability to think clearly or competently! 5/5- I feel very confident in my ability to recognize and identify my feelings. We have focused on self-awareness extensively in this program, and I more than recognize and understand the importance of this for overall well-being and my practice as a nurse. Standard 2
1.05- identifies and prioritizes client needs
Will utilize my head-to-toe assessment skills as well as communication to identify client needs. Reviewing resources on Stenberg med- surg clinical site such as reassessment of the hospitalized patient and head- to-toe assessment as well as the nursing process.
Will utilize critical thinking skills in determining priority of client needs- as well as problem solving for appropriate organization.
Will read chapter 6 from foundations of nursing p. 121- 136, nursing process and critical thinking focusing on a systemic method, problem solving, and time management
Will work on identify/prioritizing needs each day on the floor with patients. Have utilized my assessment and communication skills, in combination with my critical thinking skills to prioritize my care and client needs for the day. I have reviewed the Stenberg resources and refreshed on assessments, reassessments and the nursing process.
Example: I have an IV medication to give at 0900 to one of my patients, and it is 0900, however my other patients pain level was rated at a 9/10, therefor I will address the pain first, as I dont want my patient sitting there in pain, and then I will do my IV medication (as I have an hour window).
Read chpt.6, and took notes on the nursing process and critical thinking, focusing on problem solving and time management. 4.5/5- I feel much more confident in this skill now. I feel like I can competently prioritize my clients needs and have my own flow of how to manage my time and day while doing so. Standard 2
3.36- Follows policy on admission, transfer and discharge procedure
Will read policies and procedures for floor 4- and determine what I would need to know for their protocol for admission, transfer and discharge. Take notes of things to remember.
Will watch a nurse who takes an admission or discharge (if possible) and identify the process/steps/routine, asking questions if need be.
Ensure to follow care plan under discharge plans, any care actions to take prior to discharge or transfer.
Ask the appropriate persons for any additional information, and seek guidance if unsure.
I did determine the protocol for admission/transfers/discharges, and definitely understand the whole process.
I observed what I could of admissions and discharges, and how the nurses went about this; i.e. paperwork, preparation, collaborating with unit clerk and doctor, etc.
I spent more time looking at discharge plans in the pt. charts, and identified different goals/plans for different patients.
3.5/5- I didnt actually get to do a discharge, admission of transfer on this floor. However I did observe the process and became familiar with the policy and protocols of the unit. I feel confident that I would be able to follow these policies when the opportunity came my way. Standard 3
5.01- Evaluates own nursing performance
After completing skills and nursing actions, I will use my critical thinking and evaluation skills to determine ways in which I could have improved on this skill, what I could do differently, and what I will do in the future.
I will continue to utilize my journal and post conference to evaluate my nursing performance and discuss what I did well and where I can improve.
Will continue to reflect on the clinical competencies and ensure I am achieving these, and thus setting future goals to do so
Will seek guidance and feedback on my performance and reflect on this.
I have been focusing more on evaluating myself after I perform a skill, and using my critical thinking as to what I could have done differently, what worked, what didnt, and what I will do next time.
Example- the catheter, I evaluated my performance, noted what worked and what I could improve on. Also the SQ butterfly injection, I evaluated myself on the first time ever doing this, and noted that next time I would just do it quicker, to ensure less pain.
Have been reflecting on clinical competencies in my journal, and really working on critically reflecting on my nursing performance. Also have set goals for the future- i.e.-get more experience/confidence with insulins and blood sugars.
4.5/5- I feel confident in my ability to evaluate my own nursing performance, as I have focused on building this skill and ensuring I am doing so. I have used my critical thinking skills in my reflections and evaluations, and have been identifying where my strengths and weaknesses are. Standard 4
7.04- Verbalizes observations to appropriate persons
Will continue to communicate with the nurse and the team lead in regards to my patient.
Will identify when observations need to be directly communicated to a doctor, and thus take this action.
Will review policies and procedures on floor 4, to ensure I know what observations should be reported to who, and ensure I am following protocol.
Will continue to actively verbalize my observations on the floor to appropriate persons, and ensure I am doing so correctly.
Will review my resources on nursing process and assessments, to ensure my observations are accurate and appropriate to be reporting.
Have really been working on and focusing on communicating with the other nurses independently, and making sure I am reporting my observations to the team lead as well as the nurse on my team.
I have identified when something is of importance and needs to be reported verbally (abnormally high BP, high temp, vomiting, blood in stool, edema in one arm, etc.).
I have noted protocols on the floor for verbalizing to appropriate persons and thus followed these protocols. (Ive made doctors notes for important matters, such as pt. left arm newly edematous).
Have reviewed resources, and ensured I am appropriately reporting.
4.5- I feel I have gotten a lot better at this within the last couple weeks, mainly gaining more confidence to speak with these appropriate persons and verbalize my observations and concerns, and discuss my patients. I will continue to build this skill!
