As a Community Services funded service provider, you are required to comply with the performance monitoring and review conditions specified in the Community Services Service Agreement relevant to the reporting year. The Annual Performance Report provides an opportunity for you to demonstrate the outcomes achieved for your client groups. The template which follows will assist you in presenting your Annual Performance Report in a standardised manner. The Annual Performance Report template records the extent of service providers performance as measured against the relevant performance measures and targets set out in the Service Agreement and its schedules. To complete the template, you will need to refer to your organisations Service Agreement and Service Specification relevant to the reporting year. Information about the Annual Performance Report template : Section 1 This section relates to your organisation and the funded service to which the Annual Performance Report applies. The information to complete this section is located in the relevant Service Agreement and Service Specification. Section 2 This section provides you with an opportunity to list the results achieved for the agreed outcomes, their key performance measures and performance targets. The background information which will assist you to complete this section is located in the funded service specification. You are encouraged to copy and paste the information on outcomes sought, key performance measures and performance targets from your service specification into the Annual Performance Report to ensure that your service results are reported for all the agreed outcomes. Section 3 This section provides you with an opportunity to reflect on any issues relating to your funded service which require a special mention. For example, you may wish to report on unexpected positive outcomes of your project; a challenge such as emergence of new client groups which presented a challenge for service delivery; or significant changes within your organisation which could have potentially destabilised your project. Section 4 Please ensure that only the appropriately authorised officer in your organisation signs off on the Annual Performance Report. Deadline for submission Annual Performance Reports, together with the financial accountability documents (please refer to the Service Agreement relevant for the reporting year) are due by 31 October of the reporting year. For any additional information please contact the Community Services Project Officer (CPO) for services managed regionally or Senior Project Officer (SPO) for services managed centrally.
