These include: gym memberships, yoga, Kids swimming Programs, Pilates, nicotine replacement therapy and weight management programs. Gym memberships, yoga, Kids swimming programs and Pilates courses require your doctor or Recognised Provider to complete a Living Well form to confirm the program is medically necessary for your health.
