ASSIGNMENT Term 1, 2013

The assessment sheet for this assignment describes the performance criteria that will be assessed in this assignment. It is the student's responsibility to ensure that all performance criteria specified for this assignment are addressed. The assessment sheet (first page of this document) must be handed in with the assignment and any re-submissions. Additional files (if required) can be located from the Cert 3 OHS Moodle site.

TASK
You are required to submit a report to management detailing the findings of a worksite audit of an IT work environment. This will be an area of your choice but you will need to consult your lecturer about the suitability of this work area before going ahead with the audit. The report you compile will need to outline the hazards that you can foresee. You will need to make an assessment of the hazards and formulate an action plan, which details short and long term strategies for controlling the risks. Your report should include the following information: List breaches of the OHS&W regulations. The specifications for any features that may have been breached. Consultative procedures to be followed to correct the breach. This must include the following: 1. Record and address any barriers that may hinder the consultative breaches 2. Include any laws that may have been breached 3. Include specific names of relevant personnel who are responsible to correct the breaches and specific dates when this must be completed. Identify safe practices or good working procedures. Provide recommendations to the management regarding improvements, which will ensure the safety and health of all involved in the area.

The layout of your report will be as follows: A cover page with the names of persons conducting the inspection, the names (and titles) of management this is addressed to and a brief explanation of the procedures such as the date the inspection was carried out, the area inspected and a brief summary of the findings and recommendations (including specific dates). The second page should contain a map of fixed work areas and include all small rooms or areas not frequently visited. Include labels and/or a legend to clearly identify workstations, furniture and other equipment. The body of the report should detail the findings of the inspection shown as a checklist consisting of the actual Safety Inspection Checklist and any other checklist that may be relevant (e.g hazard identification and the Corrective Action Checklist). The report should also discuss the findings of the worksite inspection including safe practice occurrences and any breaches observed. It should also include the workplace consultative procedure to be followed to correct any breaches (Include specific names of relevant personnel who are responsible to correct the breaches and dates). The final section of the report should provide recommendations on control of the hazards identified in the worksite inspection. Please upload this to the TAFESA Moodle site with a cover sheet.
