SHE METHODOLOGY

1.0

INTRODUCTION This Health, Safety and Environment Plan has been compiled for the upgrading of approximately 104Km of road D104/D791 Chipata to Mfuwe in Eastern Province of Zambia, from an existing gravel road to a bituminous surfaced road. The Road Development Agency (RDA), appointed Brian Colquhoun, Hugh ODonnell and Partners (BCHOD) Consulting Engineers to design and supervise the proposed road upgrade, who for the purpose of easy monitoring of construction activities in Safety, Health and Environmental aspects, requested the Contractor, Sable Transport and Construction Ltd to compile and submit this SHE. The pre-construction information provided by the client and BCHOD provides guidelines, on which this Site Specific Implementation Plan has been compiled. This Health and safety plan is a dynamic document that will change and develop through the Project. It shall be read in conjunction with other Method Statements that the Contractor shall submit to the Engineer, and especially the EMP Methodology. All persons working on or visiting the site will be made aware of the availability of this plan and its contents and the document will be clearly displayed and made available to everyone on site.

2.0.

OBJECTIVES OF THIS METHOD STATEMENT The purpose of this SHE is to ensure that all potential life-threatening and disabling situations resulting from construction activities of the project are eliminated, and that all occurrences of all incidents which could lead to personal injury, damage to or loss of property and plant, and damage to the environment are minimise, and that the required preventive measures are all implemented. The document also provides a basis for continued monitoring of the construction phase by all parties involved including interested and affected parties in a meaningful way.

3.0. 3.1

ASSUMPTIONS General: It is assumed that final design report and its revised version (on realignments) adequately took into considerations that the upgraded road is created to largely prevent and reduce accidents as a result of good standards of design and remedial measures adopted to improve the existing road. Existing Environment: It is assumed that the availability of existing drawings and information has been extensively researched by the client and design team, and that all relevant information has been included in the preconstruction information and design drawings. ROLES AND RESPONSIBILITIES The implementation of this SHE methodology shall require the involvement of both the Engineer and Contractor, each fulfilling a different but vital role to ensure sound Jobsite Risk Assessments and Management during the construction phase.

4.1.

Engineer: In addition to general project management, the Engineer shall be responsible for ultimate on-site decisions regarding environmental, health and safety management, including;

1. Organising the project and supervising to ensure that work is carried out to agreed standards of safety and quality, with minimum impact on environment and risk to workers, equipment and materials. 2. Coordinating the activities of the contractor, and monitoring to ensure that the legislation, site rules and other instructions are being complied with. 3. Being familiar with the details of contractors Emergency Plan (Appendix VIII). 4. Ensuring that the necessary environmental, health and safety authorizations and permits have been obtained before commencement of works. 5. Reviewing and approving the Contractors method statements. 6. Assisting the Contractor in finding responsible solutions to problems relating to safety, health and the environment where necessary. 7. Ordering the removal of person(s) and/or equipment not complying with the SHE specifications. 8. To monitor and review the on-site risk management and implementation of this SHE by the Contractor. 7. Enforcement of all the recommendations of the SHE and ensuring Contractor complies.

4.2

The Contractor: The Contractor will appoint a Safety Officer to perform environment, health and safety functions on site concurrent with his other duties. The Safety Officer shall be expected to impart Safety and health conscious to all construction workers involved on the project. He shall be responsible for overseeing the Contractors internal compliance with the SHE requirements and that the environmental, health and safety specifications are adhered to. He shall also be responsible for keeping detailed records of all site activities that may pertain to the SHE The Contractor will ensure the following obligation; a) To continue developing this construction phase Health, Safety and Environment Plan into a working Project document by ensuring that it contains all the necessary information. b) Make clear to all sub-contractors and employees on the site (through site inductions) both the clients requirements and the Contractors site specific health and safety rules (Appendix II). Project safety information will be disseminated through site inductions and bi-weekly briefs or talks with employees and sub-contractors representatives. c) Take reasonable steps to ensure that all employees co-operate as far as is necessary to enable each of them to comply with relevant statutory provisions as directed by the Engineer. d) Restrict site access, whenever it is necessary to avoid risks, and allow only authorized persons. e) Advise the Engineers representative of any discoveries or proposals regarding design matters. f) Engage competent and experienced personnel or sub-contractors as far as is reasonably practicable. g) Monitor the health and safety performance of persons and subcontractors working on the Project. h) Secure all information that will be required for inclusion in the handover. i) Maintain the provision of training and safety information to all those on site that may suffer risk to their own health, safety and welfare whilst working on the Project. j) Encourage an open door policy and blame free safety culture in the reporting of hazards and useful work practices. The requirement of all employees to look after their own safety and not engage in activities which will put others at risk or cause them harm will be emphasised.

It is the intention of the Contractor that: Activities shall be carried out in accordance with Contract provisions to include the design and management regulations. Facilities will be provided for both contractor and Engineer on safety and health matters, and information arising which has a health and safety risk implication will be disseminated to those who may be so affected. Management of the Project shall include the encouragement and maintenance of the co-operation between all employees and individual project parties (i.e Engineer and Contractor) working on the Project. Expert advice and assistance will be obtained where necessary to discharge obligations and duties identified within the Contract. The works shall be completed in accordance with the quality and safety standards specified, to programme and budget as contained in the Contract.

4.3

Safety Standards and Objectives for the Project The Contractor will in undertaking the works (briefly outlined in 3.2 above and as detailed within the specification of works) aspire to; Achieve zero fatalities, zero permanent disabilities and improve safety performance as the project progresses. Comply with all current Health and Safety Legislation and Approved codes of practices by the Engineer. Work with and advise the Client in his aspiration to provide a better environment for his employees. Maintain safe and unimpeded access and egress from the site, particularly for emergencies, and minimize the disruption to neighbouring communities, (both vehicular and pedestrian). Identify and address all risks arising from construction activities to include fire. Police and co-ordinate, through an appointed Site Health and Safety coordinator (Safety Officer) the use of safe procedures, tools, plant, equipment and the appropriate use of Personal Protective Equipment (PPE). Work with the Engineers to maintain safety and site co-operation. Appoint a Safety Officer to carry out safety audits and inspections. Maintain adequate levels of welfare facilities for the work force. Assess the impact of site activities on the environment and manage them to minimize it as contained in the EMP Methodology.

Identify and provide health and safety training to promote awareness of safety of self and others where necessary.

5.0

MANAGEMENT TEAM ORGANISATION & RESPONSIBILITIES The Contractor shall select a team who has a wealth of experience with this type of Project covering all disciplines which will be deployed in order to achieve the Project goals. The team function shall be to; Integrate with the Engineer to achieve the best solutions for the Project Work as one with the Engineers Project Team to achieve the objectives and safety, health and environmental goals embraced by the Client and published in this document. Develop a team philosophy with employees and sub-contractors so that there is mutual understanding as to what must be achieved to meet the Clients expectations. Employ contractors depth on management knowledge and previous experiences of undertaking similar Projects to achieve the best outcome for the Client. Bring the Project in on time, and budget, to meet the clients aspirations. Proposed Project Organisation and Personnel The Contractor shall adopt the Organisation Structure in Appendix I for this Project. 5.2 Responsibilities 5.2.1 Managing Director

e) Make adequate financial provision for implementing the health and safety policies. 5.2.2 Contracts Manager

a) Promote a positive health and safety culture by creating an interest and enthusiasm for health and safety in all activities and set a good example (including wearing appropriate protective clothing and equipment). b) Be aware of the appropriate statutory health and safety requirements affecting the operations of the company. c) Ensure appropriate health and safety training is given to all employees as necessary. d) Ensure health and safety factors are considered at all stages in the work process and sufficient resources are made available to maintain safe working methods and equipment. e) Ensure a competent Safety Officer is appointed for the Project. f) Ensure compliance with the company policies and procedures and reprimand any managers and employees for failure to satisfactorily discharge their responsibilities for health, safety and environmental issues. g) Allocate sufficient resources (including financial) to enable the Health and safety policies and procedures to function effectively. 5.2.3 Site Agent/Materials Engineer

a) Promote a positive health and safety culture by creating an interest and enthusiasm for health and safety in all activities and set a good example (including wearing appropriate protective clothing and equipment). b) Ensure that all managers and employees under their control have received the required training in health and safety. c) Ensure that all managers and employees under their control are aware of their health and safety responsibilities. d) Ensure compliance with the company policies and procedures and reprimand any managers and employees for failure to satisfactorily discharge their responsibilities for health and safety. e) Co-operate with the Engineers and other Advisors in health, safety and environmental matters on site. f) Ensure all reportable accidents/incidents are immediately notified to the Engineer and other appropriate persons.

g) Where responsible on sites, ensure a site health and safety coordinator is appointed for each site. h) Where responsible on sites, ensure that inspections and audits are carried out as required. i) Where responsible on sites, ensure that offices/sites are laid out and maintained to secure site safety, clear access/egress/emergency routes and that welfare facilities are well maintained. 5.2.4 Site Engineer/General Foreman

a) Promote a positive health and safety culture by creating an interest and enthusiasm for health and safety in all activities and set a good example (including wearing appropriate protective clothing and equipment). b) Ensure the Safety Officer undertakes his duties and support him. c) Ensure everyone on site is aware of their responsibilities with regard to health, safety and environment. d) Ensure compliance with the company policies and procedures and reprimand any employees for failure to satisfactorily discharge their responsibilities for health and safety. e) Co-operate with the Engineers and other Advisors in health, safety and environmental matters on site. f) Ensure that inspections and audits are carried out as required. g) Ensure that a risk assessment has been carried out for any equipment, work activity or substance hazardous to employees health and safety. Ensure appropriate control measure, training, instruction, protective clothing, etc., have been provided and that there is supporting documentation. h) Ensure that all plant/equipment/machinery (including office equipment) is safe fitted with any necessary guards or safety devices and is serviced and maintained as recommended by the manufacturer (including testing of electrical safety as required). i) Ensure that site/offices are laid out and maintained to secure site safety, clear assess/egress/emergency routes, and that suitable welfare facilities are provided and maintained. j) Ensure there are sufficient and suitable first aid facilities on site, and that notices giving the location of the nearest hospital with accident aid emergency, facilities are clearly displayed in appropriate locations. k) Ensure all reportable accidents/incidents are immediately notified to the appropriate persons.

5.2.5

Safety Officer

a) Promote a positive health and safety culture by creating an interest and enthusiasm for health and safety in all activities and set a good example (including wearing appropriate protective clothing and equipment). b) Assist the Site Agent in the safe running of the site and ensure that the site is organized so that work is carried with minimal risk to employees, sub-contractors and others affected. c) Ensure the adequacy of Contractors risk assessment and method statements for all activities and oversee any high risk activities or temporary works. Act as a point of contact for contractors whose activities will impact on others operating on or using the site. d) Ensure permits/approvals to work are issued, recorded and signed off for all appropriate works. e) Ensure that induction procedures are in place and implemented for everyone who works on site or visits the site and ensure that emergency arrangements are included within the induction and displayed on the site notice boards. f) Carry out a minimum daily walkover of the site and ensure that all hazards and risks are being addressed. g) Keep all registers/records/reports in detail, up to date and within a safe place. h) Ensure that all statutory notices are displayed on site, notice boards and that First Aid facilities, and Fire-fighting equipment are maintained. i) Ensure safety information is provided for everyone who works on site or visits the site, obtain feedback of any health and safety issues and inform Management. 5.2.6 Sub-Contractors

All Sub-Contractors will operate in accordance with the Main Contractors procedures, site specific rules and Clients requirements as explained both with the site induction process and in the Contractors documentation. All Sub-Contractors shall co-operate with each other to facilitate the safe operation of site activity. All Sub-Contractors either failing to meet those site rules that are governed by zero tolerance or those failing to comply with safe

practices/provision of safe tools and equipment will be asked to remove themselves and their equipment from the site. 5.3 Site Safety Policy Statement

The Contractor regards the provision of a safe and healthy working environment on construction sites as a principal objective. The objective will only be achieved by the co-operation of the contractors employees, sub-contractors, the Client and Engineers representatives. Co-operation will be expected to be at all levels within these different organisations through the structures established under the Contract. The Contractor will collaborate with all parties to provide the organisation, and advise on resources to meet this commitment so far as is reasonably practicable. Authority to implement this policy will be defined for all those who have a responsibility for healthy and safety. The contractor has already established a series of management procedures to ensure that healthy and safety issues retain a high profile during all stages of the Contractors activities. It is the responsibility of the Contractor to: 1) Sustain and carry out this policy by all means at their disposal. 2) Provide adequate safety and job training for all employees with particular attention to special safety training where appropriate. 3) Comply with the requirements of the relevant Legislation to undertake risk assessments of all activities and to ensure that safety systems of work and a safe working environment are put in place. 4) Ensure that the operations of the Contractor are carried out without risk to the health and safety of third parties and the environment. The Contactor will seek to maintain a constant interest in all aspects of safety by effective consultation with all parties concerning hazards and incidents which affect health and safety at work and to prevent any adjustment or damage to plant and equipment which may create a hazard. All Managers and Supervisors on site are responsible for observing Contractors policies and procedures and for ensuring that at all times they work in a manner consistent with the safety of themselves and others.

The effectiveness of health and safety measures will be monitored continuously in order to ensure that both policy and practice are appropriate at all times to the activities of the Contractor. All those involved in the construction phase have a statutory duty to comply with this SHE and to provide the Contractor with any information which they have, which is needed to keep the plan up to date. Anyone wishing to seek advice on compliance should contact the Contactor. 6.0 INFORMATION ABOUT RESTRICTIONS WHICH MAY AFFECT THE WORK The following points will be noted at all times; 6.1 Deliveries All deliveries will be met and reversed on site by a competent banksman Deliveries will be managed to run with the progress of works so that storage is kept to a minimum. Deliveries will not be unloaded adjacent to working area without prior agreement from the Safety Officer. Traffic /Pedestrian Routes

6.3

Clear access for emergency personnel shall be kept at all times. All accidents will be reported and details entered into the Site Accident Log (Appendix III) and the HSE Statistics Report (Appendix VII). Details of the accidents will be forwarded by the Site Agent to relevant Authorities, the Engineer and other appropriate persons, in accordance with the Contractors Accidents Reporting Procedure. Site specific emergency arrangements, as highlighted in Appendix VIII and in item 8.12 of the EMP, will be explained as part of the Site specific induction. The emergency contacts and details will also be posted on the Safety Notice Board.

A designated site Fire Officer will be responsible for carrying out a fire risk management assessment and make recommendations; he/she shall monitor the site for compliance on an ongoing basis. These observances will be continually reviewed and developed as the project progresses. The dissemination of changes to safety information shall be through the Safety Officer to all site employees and subcontractors.

7.0

7.2

The Design

The table below highlights the significant hazards identified by the Contractor as a result of the design and relevant information has been included in the pre-construction information and design drawings. The Contractor will manage these hazards during the construction process and will ensure that this section is kept up to date at all times.

8.0

Identification and Effective Management of Activities with Risks to Health and Safety 8.1 Risk Assessments

The Contractor shall conduct and develop risk assessments prior to commencement of any works as part of the establishment process on site. All subcontractors will also be required to submit an overall Project Method Statement and details of high risks activities or, activities which will impact on others working on site. Additional periodic risk assessments will be made whenever required and as risky activities arise. Where these activities affect other operations the relevant assessment and information shall be made known to all affected. Risk assessments will be used to identify operations or areas where permits to works are required as part of Contractors control procedures. The Safety Officer shall be responsible for the issue of permits to work for the following activities; a) b) c) d) e) f) g) Surfacing with hot bituminous materials (Hot works) Working with Density- Moisture Gauge (Troxler) Operating heavy plant and equipment. All personnel will undergo an induction both on Contractors Safety Procedures and on site-specific safety before being allowed to work on site. Subcontractors will also undergo a site specific induction before being allowed to work on site and will take responsibility for the safety and task specific training of their own personnel.

The Contractor will carry out random site inspections and audits jointly with the Engineer and produce a report. Action on all non-conformances will be agreed with the Engineer. Both the Contractor and the Engineer will monitor the activity of the Contractor; non-Conformances identified by the latter will result in the issuing of a Site Instruction requiring immediate action. Anyone found not to be working safely will be removed from site. 8.4 Safety Meeting

Safety matters will be on the agenda of the following meetings; Site based daily meetings (between appropriate parties) will cover urgent safety issues to ensure everyone on site knows what is going on that day. Specific hazards will be highlighted here as a reminder. Site based Contractors weekly or bi-weekly co-ordination meetings where actions on safety will be covered. These meetings will also be used to co-ordinate the works between interfacing work groups to ensure all safety issues are covered. Toolbox talks will be given to site employees on an ad-hoc basis by selected Contractor staff where it is deemed necessary to hold a talk on a particular aspect of safety.

9.0

ARRANGEMENTS FOR VETTING CONTRACTORS, SUPPLIERS OF MATERIALS, MACHINERY AND PLANT WITH REGARDS TO HEALTH AND SAFETY Arrangements shall be made for all Sub-Contractors and Suppliers of materials, plant and equipment to issue them with a pre-qualification questionnaire to assure the Contractor that they are competent Contractors, prior to their start on site.

Noise Operations shall be subjected to noise assessment if required and control measures implemented. The Contractor shall implement best practicable means to minimise noise in accordance with current statutory regulations and local requirements. The Site Agent will instruct the stoppage of any operation he deems to be unreasonably noisy. The Contractor will ensure that Personal Protective Equipment (PPE) complies with Noise at work. Cognisance shall also be taken of local authority noise restrictions on permissible working hours.

10.0 MANAGEMENT MEETINGS AND INITIATIVES The Site Agent shall be a point of contact for discussing and taking view on Health and Safety for all persons working on site. In addition each Site will have a dedicated appointed Site Health and Safety Co-ordinator. Sub-Contractors coming on to site for the first time will meet with Safety Officer to discuss emergency arrangements and procedures, the provision and use of plant and equipment to be used by the subcontractor and discuss how the subcontractors works will be carried out to avoid risks to others. Various meetings will be held throughout the course of the Contract, as detailed below, minutes from which will be circulated to the appropriate parties, including the Engineer. All construction works, which involve design proposals or change will be related to the Engineer as required by the Contract. 12.0 EMERGENCY ARRANGEMENTS a) The emergency arrangements for every site will be explained to inductees as part of the site induction and details posted on the Site Safety Notice Board. b) Every site will have an appointed Site Health and Safety Co-ordinator, First Aider(s) and Fire Officer. c) A Site Emergency will be one or more of the following; fire, accidents, or any other incident deemed an emergency by management. d) In the event of a fire emergency, all employees will be required to proceed immediately to one assembly point and remain there until accounted for and given permission to leave. e) In the event of any of the emergency services being required to attend site, the site Health and Safety Officer must coordinate and give details of any flammable materials/chemicals on site. 12.1 Accident Procedure including Reporting a) All accidents on site shall be reported to appropriate authorities, including the Engineer, even if no medical attention is given. This will also include accidents involving visitors to the site, delivery drivers etc. The Site Agent shall oversee all entries made in the Site Accident log which will be kept in the site office. b) Persons discovering the accidents will be required to raise the alarm verbally through two way radios operating on site or by use of mobile telephone. c) In the event of an incident/accident, all personnel will be accounted for. d) The Safety Officer will check total headcount and if necessary mobilise an emergency rescue team. e) Following an accident or dangerous occurrence a complete investigation will be carried out and a report given to the Engineer. 12.2 Fire Prevention The Contractor shall carry out works so as to eliminate risk from fire. a) Fire extinguishers, fire notices, fire detection, etc shall be provided for all working areas. b) Materials and rubbish stored on site will be kept to a minimum.

c) Rubbish will be cleared from site on a regular basis and site working and access areas kept clean. d) Where flammable material must be kept on site, they will be stored in fireproof containers away from fire exit routes and potential fire hazards. e) A No Smoking rule will be imposed in all working areas, storage areas or wherever there are fire hazards. f) Employees will be allowed to attack the fire with the appliances provided only if they are trained to do so and as long as there is no risk to themselves or others. g) If the fire cannot be safely tackled then all persons shall leave the site to nearest safe place and report to the person in charge of the assembly point, normally the Safety Officer. h) The person in charge shall make a record of all those who were present on site and ensure that the emergency services have been called. The procedures will be developed as necessary and identified during site inductions. 12.3 Emergency contact Telephone Numbers A list of emergency contact telephone number is included in this plan as appendix VIII. These details shall be displayed in a prominent position in the site office. 13.0 MAINTENANCE OF WELFARE FACILITIES

The following facilities will be available on site and cleaned daily; a) b) c) d) Toilet and bathrooms (ablution block) A source of clean drinking water A first Aid box. (which will be kept in the site office). Sufficient and suitable firefighting equipment

The Contractor shall provide his own First Aider/ appointed person and their own first aid box, all accidents or incidents will also be reported to the Safety Officer.

14.0 PROVISION OF INFORMATION 14.1 Information for Co-ordination and co-operation of employees Operatives and subcontractors will be informed at engagement stage of; Risks to their health and safety arising from the environment in which the project is to be carried out. Copies of any relevant Methodologies or risk assessment for activities which impact or have a possibility of impact on individual operatives, sub-contractors which will be issued to them and all risks highlighted for discussion to include in the development of this dynamic Safety Health and Environment Plan. Initial site induction training which every operative will undergo through. Need for Maintenance persons etc. visiting site for a short duration to receive a short induction. These persons will only be allowed onto site if they are dressed appropriately for the prevailing site condition. Need for subcontractors representatives to attend fortnightly Site Meetings, so that co-operation and communication are encouraged.

All key operatives, subcontractors will be issued with this Construction phase Health, Safety and Environment Plan, as well as being inducted. 14.2 Training Toolbox talks will be provided for general health and safety matters on a bi-weekly basis as and when requested. Specialist training in the use of certain tools and equipment shall be secured. All operatives and subcontractors will provide information on the suitability of the training undertaken by their operatives as part of their Health and safety documentation prior to commencing any works. All subcontractors will be required to carry out toolbox or task health and safety talks with their employees on key points of Health and safety. Where the need arises specific toolbox or task talks will be carried with the employees concerned.

All Toolbox talks and site briefs will be recorded on the appropriate form. 14.3 Health and Safety Information Employees who operate plant and machinery must have documented evidence that they are qualified to do so prior to commencing the work. 14.4 Registers and Documentation The following will be available on site; a) Accident Log (Appendix III) b) Record of inspection required (Appendix IV) c) Waste transfer notes d) Hazard data sheets, risks assessment e) Electrical equipment registers maintenance checks and records. f) Plant inspection records g) Works Methodologies h) Health surveillance (Appendix VI) i) Emergency procedures (APPENDIX VIII) j) Record of site inductions 15.0 CONSULTING AND COORDINATING THE VIEWS OF PEOPLE The Contractor will carry out regular consultations with the workforce on matters of health, safety and environment on site. The safety plan and other health and safety related matters will be formally reviewed at regular meetings which will be held on site. Attendance at these meetings will be made obligatory for Site Engineers, General Foremen, Surveyor and Works Foremen. Site safety issues will be coordinated by the Safety Officer with the support of the Site Agent. A team approach to safety will be encouraged with suggestions from all site personnel welcome. The results of risk assessment and investigations of accidents and dangerous occurrences will be reported to the whole workforce by site briefing to maintain the level of safety awareness.

Temporary works design and construction will be coordinated by the Site Agent. For elements of works designed by the Contractor, the Site Agent will coordinate the design process and liaise with the Engineer. Approval of these designs will be by the Engineer. 16.0 SITE RULES The specific site rules, Appendix II, will be notified to all employees and this will be reinforced through site inductions. 17.0 HIV/AIDS MANAGEMENT ACTIVITIES The Contractor will embark on two action plans in order to manage HIV/AIDS within the workforce and the Project Area Communities as follows; 17.1 Workplace HIV/AIDS Action Plan The Contractor will ensure that the risk of HIV/AIDS within its workforce is reduced significantly through the implementation of such policies and programs as; 1. 2. 3. 4. 5. 6. 7. 8. 5. 6. Assigning internal responsibility; Forming a Workplace HIV/AIDS Management Committee; Developing an HIV/AIDS policy; Implementing education and prevention programs, to create awareness, address gender issues, provide training, Supporting peer education and counseling programs. Promoting condom use and distribution. Overseeing occupational health and safety procedures. Encouraging voluntary counseling and Testing (VCT) and enable prevention and treatment of STDs; Supporting for care and treatment, of opportunistic infections and STDs, in the form of ARVs and as a wellness program; Monitoring and surveillance.

17.2 External Outreach HIV/AIDS Action Plan The Contractor will further reduce the risk of HIV/AIDS by conducting sensitisation programmes in and with communities around the Project area. This will include coordinating awareness and prevention measures with community and external stakeholders and establishing joint programs that effectively mobilize resources for the reduction of the impacts of HIV/AIDS.

The Key activities will include: Forming a HIV/AIDS Community Outreach Committee; Developing an Outreach Plan, to coordinate awareness and care programs and monitoring and surveillance initiatives; Building partnerships. Ensuring that Management is represented at the Chipata District Aids Task Force (DATF) by one senior member of staff. 17.3 Monitoring and Evaluation In order to enable constant development and improvements to the plan over time, the Contractor shall monitor and evaluate impacts and outcomes of the above HIV/AIDs campaign activities and shall report to the Engineer. This will mainly follow two general objectives: 1. Monitoring of specific situations arising from Plan implementation, and of compliance with objectives and methods set out in the Plan; 2. Evaluation of the mid- and long-term impacts of the Plan on Contractors workforce and Project Area communities. 18.0 THE HEALTH AND SAFETY FILE Arrangements will be made for all stakeholders to pass on all relevant information pertaining to safety and health, for inclusion in the Contractors Safety and Health file. The Contractor will also collect and collate the requisite information from each subcontractor and issue it to Engineers Representative two weeks prior to practical completion. 19.0 MONITORING OF HEALTH AND SAFETY PERFORMANCE 19.1 Monitoring The Safety Officer will actively monitor health and safety performance on site. He will carry out daily inspections of the works and will highlight to the subcontractor concerned, any breaches or bad practice that may be a danger to themselves or others working in the area. All non-conformances will be closed out by a re-visit. The Contractors Management will make periodic safety tours and consult the site Management on any corrective action needed.

Monitoring of site Health and Safety will also be discussed at the fortnightly Contractors liaison meetings where related subject will be raised, discussed and actioned. All subcontractors and visitors will have a responsibility to comply with the site specific rules and ignorance will not be considered as defense. 19.2 Project Review On completion of the Project an internal review will take place to assess how the standards set compare with those actually achieved in order that any short comings can be addressed in future projects.
