Emergency Evacuation
1. Patient to be brought to Babo / Sorong / Ambon by locally available means of transport. (International SOS will direct onsite and off-site logistics as available). 2. International SOS to organize onward mode of transport by ground or air ambulance from airport to nearest hospital for stabilization or continued care. 3. Travel necessities (passport, visa, documents, clothing etc) to be organized by local management. 4. International SOS to liaise with local providers to facilitate admission under appropriate specialist and immediate care. 5. International SOS to guarantee payment at hospital/s to expedite admission. 6. International SOS to monitor care and update Dimas Utama management on condition of patient and next medical steps as required.

Urgent Referal
1. Patient will stay on Site until appropriate transfer can be arranged. 2. Company to move patient by next available transport to nearest appropriate medical facility. 3. Travel necessities (passport, visa, documents, clothing etc) to be organized by local management. 4. Inpatient admission required; a) International SOS to organize and liaise with local providers to facilitate admission and further care. b) International SOS to arrange guarantee of payment at hospital if required. c) International SOS to monitor Care and update Dimas Utama management as required. Dimas Utama, PT, operating partner of Genting Oil, is conducting drilling exploration located in Kasuri, Teluk Bintuni, Papua, Indonesia; nearest city Sorong and nearest recommended city is Ambon. Immediately call International SOS Jakarta Alarm Centre in the event of serious illness or injury. If for any reason, contact cannot be made with International SOS Jakarta Alarm Centre, the secondary International SOS Alarm Center can be contacted (or any other International SOS Alarm Center as necessary). Your message will be passed to the appropriate Alarm Center. NOTES: Due to the progressively worsening nature of some medical conditions and the logistical complexities of remote Site operations, call EARLY. At the time of the first call, a two-week travel history on the patient must be communicated, especially but not only if the patient has a febrile or respiratory tract illness.

3.1.2. HOW TO ALERT

Name of the patient (and the company the patient works for if not Dimas Utama, PT). Nationality of the patient and location of patients passport. Brief description of the medical problem.

NOTES: Use Appendix 1 Medical Report Form and fax or mail to International SOS.

3.1.3. STABILIZATION ON SITE


The International SOS Alarm Center Coordinating Doctor (CD) will advise on how to stabilize patient and to prepare them for medical evacuation if required. Site medical team must manage the patient for the duration prior to movement off site. This period will depend on the available rescue logistics. A recommendation regarding the need, mode and destination of evacuation will be undertaken by the International SOS Coordinating Doctor (CD) in conjunction and will be based on patient's condition, location and local logistics. 7

3.2.

AUTHORIZATION

Dimas Utama, PT Authorized Person (AP) must confirm their agreement and authorize any cost that may be incurred for the evacuation and for any subsequent hospitalization costs. Refer to Authorized Person (AP) listing in Section 5.1.

3.3.

EVACUATION

Evacuation will occur in one or two phases: Phase One: Primary Evacuation If evacuation is recommended, the patient should be transferred using the Site medical team to the nearest medical facility. Mode of transport will be selected on a case-by-case basis, using either the sites transport assets (the fastest response), or third-party providers appropriate for the case in the area. The nearest medical facility may not be and in remote areas usually will not be the medical facility best suited for the patients definitive treatment. International SOS may have administrative agreements with these hospitals and providers to assist with the admission of patients and guarantee of payments but this does not mean these hospitals are credentialed by International SOS to international standards or the standards of International SOS. International SOS members are always strongly advised to contact the Intl SOS designated Alarm Centre as early as possible so the most appropriate medical facility for both first care and definitive care can be advised on a case by case basis. Medical facilities recommended only for initial admission and stabilization prior to onward transfer for the more serious cases may be appropriate for definitive care of less serious cases but in any case only represent the best locally available option. Onward referral to a regional centre of medical excellence will be recommended on a case by case basis by the International SOS Medical Team. International SOS cannot be hold liable for any consequences resulting from treatment received at any hospitals.

Phase Two: Secondary Evacuation Secondary evacuation is the movement from the primary receiving facility to a medical facility in another location for the purposes of providing definitive care. The most common modes of secondary evacuation depend on medical and logistical criteria as well as resources available: 1) Air Ambulance: Air ambulance directly to nearest center of medical excellence if recommended by the International SOS Coordinating Doctor (CD). This option will be recommended for serious medical condition if adequate medical care cannot be provided at local facility.

2) Commercial Carrier: Patient transfer on a scheduled flight if available and if patient fit to fly (to be decided by the International SOS Coordinating Doctor (CD) and the Treating Doctor). International SOS will facilitate the travel arrangements, including airline medical clearance and medical escort as necessary.

SITE SPECIFIC EVACUATION SCENARIOS In case of emergency, patient will be brought to Sorong as a nearest point of stabilization or directly transfer to Ambon as nearest recommended city and will be transferred to nearest medical facilities (please refer to section 4.1.1). 8

Evacuation from Site to Sorong by helicopter: Helicopter origin : Sorong Helicopter type : Bell 412 Helipad dimension : Rated for Bell 412 Night landing possible : NO Aviation Navigation Beacon / NDB or similar : NO Seating capacity : 12 passengers (seats can be detached and replaced with a bracket for stacking 3 stretchers) Activation time : + 2 hours Flight time : From Site to Sorong: + 2 hours NOTES: Even in a medical emergency, we may not be able to move patients across a border if they don't have a passport and entry / exit visa; because immigration authorities won't allow; so it is a good idea in a patient's interests that he always has his passport with him (and if the patient is incapacitated, someone else on board needs to know where to find the passport in an emergency, and the passport is not locked away inaccessible). Even in a medical emergency, a country may not allow a patient entry if the country immigration regulations do not allow for an emergency visa for certain nationalities. All emergency responses are on a best effort basis; if a helicopter is not available, if the weather not allow safe flying or a safe voyage, if an airport is closed due to runway maintenance, absence of lighting, staffing restrictions, if a particular aircraft is not available because the pilots have exceeded their safe flying time, there may be unavoidable delays. Government health regulations / laws must be followed; if a patient is sick with an infectious disease and health authorities do not allow the transport of the patient either locally, regionally or internationally, then that is also force majeure. For a secondary medical evacuation, if you want us to move the patient to a place that speaks his language or where he is culturally more comfortable, even if it is not the nearest centre of medical excellence, we will do that upon request, as long as it's medically safe. If the patient has first been moved offsite to a local hospital, secondarily transferred to a regional centre of medical excellence, and finally wants to go back to the home country, we can do that on a normal aircraft if the condition permits and the airline allows.

3.4.2. INTERNATIONAL SOS DUTIES


Provide medical advice and assistance over the phone. Recommend whether patient should be treated on Site or be evacuated directly to the nearest suitable location. Liaise with Dimas Utama, PT to monitor the situation and coordinate the medical evacuation. Confirm to Dimas Utama, PT the estimated mission timings, organize medical assessment / admission in the appropriate medical facility and to guarantee all payments on behalf of Dimas Utama, PT based on approval from Authorized Person (AP). Continue to medically monitor and update Dimas Utama, PT at each stage.

Client to please provide necessary detail on how and where transport assets under the clients control or that the client is aware of i.e. any and all site transport assets, can be mobilized to support patients movement off site, as well as the capacity of a particular vehicle for a patient or patients. 2. AIR: COMMERCIAL CARRIER

There are several commercial flights with daily schedule from Sorong to Jakarta and vice versa by Garuda Indonesia Airways / Merpati Airlines and Ambon to Jakarta and vice versa by Garuda Indonesia Airways / Lion Air / Batavia Air. However, none of these airlines (except Garuda Indonesia Airways) will accommodate for the domestic sectors a patient on a stretcher and / or a patient who requires oxygen, or a patient who cannot use the seat in the full upright position for takeoff and landing. If stretcher or oxygen is required on Garuda Indonesia Airways, this may take 48 72 hours to arrange during working days and may not at all be available on weekends and public holidays. 13
