19. A 37-year-old engineer was started on diuretics three months ago for
hypertension (180/110). A few weeks later he was admitted to A&E
with vomiting, acute vertigo and gross incoordination. On examina-
tion there was bilateral nystagmus.
20. A 78-year-old hypertensive woman is referred for fluctuating level
of consciousness. On examination her blood pressure is 175/110,
fundus examination shows some hypertensive changes, her reflexes
are bilaterally brisk and her plantars are upgoing.
21. A 26-year-old presents with a week’s history of headache and
diplopia. On examination she is obese and fundoscopy shows bilat-
eral papilloedema.
22. A 28-year-old diplomat presented with sudden blurring of the left
eye. A week earlier he noted progressive clumsiness of the right
hand. A few months earlier he had had an episode of left leg stiff-
ness that resolved spontaneously.
23. A 19-year-old soldier is brought semiconscious into A&E. He has
been complaining of severe headache and vomiting of one day dura-
tion. On examination there is widespread maculopapular rash all
over his body.
24. A 36-year-old previously fit man developed a flu-like illness which
was followed 2 days later by burning pain in both legs. He later
developed urinary retention and diplopia. On examination his
straight leg raising test was positive bilaterally, ocular mobility was
limited bilaterally but sensation was intact.

52. The most common virilising tumour of the ovary. Secretes andro-
gens.
53. Large multi-cavity ovarian cyst, filled with thick fluid. It may reach
a huge size occupying the whole peritoneal cavity.
54. Single-cavity ovarian cyst, filled with watery fluid. Often bilateral.
Potentially malignant.
55. Small, solid and hard, white benign tumour. Usually unilateral. May
be associated with ascites and pleural effusion.
56. May contain sebaceous fluid, hair and teeth. May be benign or
malignant.
57. Causes excessive oestrogen production. May occur at any age caus-
ing precocious puberty in children, metrorrhagia in adults and post-
menopausal bleeding in older women.
