ADL-USER APPLICATION
[To be filled by graduate students, undergraduate research students, other research students and contractually limited
employees who may use the Applied Dynamics Laboratory (ADL) testing facilities {Main floor, basement, and the
mezzanine}] - (submit to ADL-supervisor Mr. Kent Wheeler for processing)
No person shall be permitted to use the ADL testing facilities unless they are a registered ADL-USER.
ADL-USER list is maintained in the lab and is available in the civil engineering departmental WEB-CT area item ADLUser information.
This form, if approved, will register the applicant as an ADL-USER for the period indicated below.

order to be able to use ADL space, equipment, and technician resources.
We prefer our users to attend Workplace Hazardous Materials Information System (WHMIS) training at least every 3
