GRUAS Y EQUIPOS DE IZAJE

1. PROPSITO
Este procedimiento identifica los requerimientos
y proporciona lineamientos para equipos de
izaje como, gras, bridas de unin de poleas,
montacargas, winches neumticos, y otros
equipos o materiales similares utilizados para el
izaje.
2. ALCANCE
El procedimiento se aplicar a la adquisicin,
inspeccin, pruebas y mantenimiento de equipos
de izaje.
3. APLICACIN
Este procedimiento es obligatorio para todas las
empresas
contratistas,
sub-contratistas,
proveedores de servicios generales, vendedores
y representantes de vendedores, visitas,
funcionarios pblicos, personal gubernamental y
cualquier persona asociada al proyecto dentro
las reas y caminos de acceso del
proyectodefiniciones
4. GENERAL
Todas las gras deben contar con registros actuales
de inspeccin que cumplan con los requerimientos
legales.
Todo el equipo y los mecanismos de izaje deben
cumplir con los requerimientos legales.
Todas las reas de trabajo peligrosas deben ser
cercadas de acuerdo con lo establecido en el
Procedimiento HSE 2.7 Barreras, Mallas y Cintas
Toda la documentacin de seguridad, informes de
inspeccin, aprobaciones, licencias, etc., deben
mantenerse por el Dpto. HSE de las Empresas
Contratistas.
5. ADQUISICION Y CONTRATACION
CRANES & LIFTING EQUIPMENT

1. PURPOSE
This procedure identifies requirements and provides
guidelines for lifting equipmentt such as cranes,
pulley blocks, fork-lifts, air tuggers and other similar
equipment or materials used for lifting.
2. SCOPE
Ultra Violet (UV) protective measures will be applied
in all areas on the project for all workers who are
exposed too UV radiation.
3. APPLICATION
This procedure is mandatory for all contractors, subcontractors,, general services providers, vendors and
vendor representatives, and any person associated
to the project within the areas and access roads of
the project
4. GENERAL
All cranes shall have current inspection records that
meet statutory requirements.
All lifting equipment / gear shall meet statutory
requirements.
All hazardous work areas shall be barricaded in
accordance with Practice HSE 2.7 Barricades, Mesh
Fences & Tape
All safety documentation, inspection reports,
approvals, licenses, etc., shall be maintained by the
Contractors HSE Department
5. PROCUREMENT AND HIRE

6. INSPECCION Y MANTENIMIENTO
El Contratista mantendr un registro de los equipos
de izaje junto con todos los registros de
mantenimiento e inspeccin. Todas las abrazaderas y
vigas de izamientos estarn identificadas y rotuladas
claramente. Las eslingas de carga, cadenas, ganchos
y grilletes estarn permanentemente etiquetadas o
marcadas con el lmite de carga aprobado.
Todo equipo de izaje (Montacargas, Camiones
Pluma, Plataformas elevadas-Man-lifts y similares)
se debern inspeccionar Mensualmente y en acuerdo
con las recomendaciones del fabricante, y recibir
una inspeccin diaria de Pre-Uso documentada por el
operador, tipo check-list. Dichas inspecciones diarias
se registrarn y se mantendrn en el equipo con el
operador.

ANEXOS
Toda Gra deber contar con una Certificacin Anual
de Gras, por una organismo competente reconocido
a nivel nacional, y una inspeccin Inicial previo a su
uso en el proyecto y cada mes posteriormente

Los accesorios de izaje desgastados o daados al
punto de que, segn el criterio de una persona
autorizada, no pueda ser reparado de manera
satisfactoria, deben ser destruidos cortndolos.
Registrando en el archivo del equipo las acciones
tomadas.
Si en algn momento se descubre que un equipo de
Izaje necesita reparacin, se encuentra defectuoso o
es inseguro en algn aspecto, quedar fuera de
servicio hasta que haya sido reparado y se encuentre
en condiciones seguras de funcionamiento.
Las reparaciones de Equipos de Izaje que sean
de reemplazo de piezas, deben ser realizadas
por el fabricante original, por uno de sus
agentes o por un fabricante aprobado por el
ingeniero mecnico responsable.
Cualquier
pieza de reemplazo debe ser idntica o
equivalente a la pieza original y se deber
obtener la certificacin correspondiente.
Cuando se reemplacen o reparen componentes
de soporte de carga, el equipo de izaje debe
6. INSPECTION AND MAINTENANCE

A register of all lifting equipment is to be maintained
by the Contractor along with all inspection and
maintenance records. All clamps and lifting beams
(Spreader bars) will be clearly identified and labeled.
Slings, chains, hooks and shackles will be
permanently tagged or marked with the approved
load limitation.
All lifting equipment (Fork-lifts, Boom Trucks,
Elevated platforms/man-Lifts, and similar) shall be
inspected Monthly and according to manufacturers
recommendation, and will receive a daily
documented Pre-Start check-list inspection by the
operator.
These daily inspections will be
documented and kept in the equipment with the
operator.
For monthly Inspection Forms for the above and
other Equipment see HSE 3.8 ATTACHMENTS
All Cranes will have a Yearly Crane Certification by a
competent organization recognized at the national
level, and an initial inspection prior to its use on the
project and every month thereafter
HSE 3.3 ATTACH 1 Monthly Crane Inspection
Lifting accessories that are worn or damaged to such
an extent that, in the judgment of an approved
person, it cannot be repaired satisfactorily, must be
destroyed by cutting, and the Lifting Equipment
register and record file amended accordingly.
If at any time any lifting Equipment is found to be in
need of repair, defective, or in any way unsafe, the
Liftying Equipment shall be taken out of service until
it has been restored to a safe operating condition.
Repairs to Lifting Equipment, other than the

replacement of parts, shall be carried out by the
original manufacturer, the manufacturer's agent or a
fabricator approved by the responsible mechanical

Toda Gra y Otros Equipos de Izaje debern
contar con la tabla de carga y toda la
documentacin de seguridad para operar el
Equipo de Izaje en forma Segura
Est prohibida la operacin de cualquier Equipo
de Izaje o sus accesorios de izamiento fuera del
diseo seguro del fabricante, o en condiciones
inseguras.
Toda Gra deber tener un operador
Calificado
con
Licencia/certificacin
aprobado
por
el
Proyecto
y
un
Rigger/Sealero
calificado
durante
operaciones de Rigging/Izaje
El Gerente de Obra del Contratista, o la persona
designada, con el Ingeniero de Rigging y
Representante/ Gte. de HSE, proporcionar los
detalles tcnicos y las limitaciones requeridas
para ejecutar de manera segura todas las
operaciones de Rigging
Toda Gra de neumticos se ubicar en suelo
estable y se usarn estabilizadores debajo de la

engineer. Any replacement part shall be identical/

equivalent to the original part. Appropriate
certification shall be obtained.
Where load-bearing components are repaired or
replaced, Lifting Equipment must be tested by
application of a Proof Load, as specified by an
approved person, and then thoroughly examined. A
test certificate stating the Working Load Limit/Safe
Working Load and the Proof Load must be obtained.
The test certificate and a record of the repair or
modification should be placed in the registration file.
7. OVERHEAD LIFTING ( CRANES AND RIGGINS )

All Cranes and other Lifting Equipment shall be
operated according to manufacturers specifications
and instructions
All Cranes and other Lifting Equipment shall have
the Manufacturers Load Charts and all safety
documentation required to operate the Lifting
Equipment safely.
The use of any Lifting Equipment or its lifting
accessories out of the manufacturers safe design, or
in a unsafe condition is prohibited
All Cranes shall have a Qualified Operator with an

approved project License/Certification and a
qualified Rigger / Signalman during Rigging
operations/lifting
The Contractor Site Manager, or designee, along
with the Rigging Engineer and the Site HSE
Representative, will provide technical details and
limitations required to safely execute all rigging
operations.
All Rubber tired Cranes will be set on stable ground
and Crane pads/blocks deployed under the outrigger
legs/pods
All track Cranes will be set on stable ground and
Crane mats shall be used as needed for safety.
The Crane Operator shall not lift any load if it is
unsafe to do so.
The Crane Operator and Rigger will share
responsibilities for the safe Operation of the lifting of
any load

8. MODIFICACIONES

fabricadas de conformidad con los estndares
nacionales o locales; el diseo modificado debe ser
aprobado por la autoridad correspondiente, diseador
o fabricante
Despus de realizada la modificacin, si se necesita
una prueba de capacidad indicando el Limite de
Carga/ Carga de Trabajo Segura, una persona
autorizada por la administracin del proyecto deber
otorgar un certificado
9. LICENCIA Y CAPACITACION DEL

OPERADOR
El conocimiento de los requerimientos operacionales
y de los procedimientos de seguridad que posea el
operador deben verificarse a travs de un examen
escrito, un examen prctico y mediante la
observacin del desempeo del operador durante el
primer mes de trabajo. Una persona competente
designada por el Gerente de Proyecto/Obra, como el
Superintendente, realizar el examen prctico para
determinar las habilidades operativas del candidato.
Este examen se realizar en cada clase y modelo de
equipo que deba manejarse. Cada operador debe
recibir capacitacin de conformidad con lo dispuesto
en el Procedimiento HSE 3.7, Licencia para operar
gras y equipos.
El expediente de certificacin del operador de equipo
debe completarse y mantenerse archivado por el
Contratista mientras se desarrolle el Proyecto El
operador recibir una Licencia de Operador de
Equipo anexa que debe conservar durante todo el
tiempo que se desempee en el Proyecto. La emisin
de esta licencia es vlida slo para el Proyecto.
10. MONTACARGAS
10.1 Inspeccin
Antes de operar el montacargas por primera vez
durante un turno, el operador debe
inspeccionarlo segn las especificaciones del
fabricante.

8. MODIFICATIONS
All modifications to lifting equipment and lifting
accessories shall be designed and fabricated in
accordance with national or in-country standards and
the modified design approved by the corresponding
design authority/ manufacturer .
After modification, if a proof test is required by
applicable standards, a test certificate stating the
Working Load Limit/ Safe Working Load and the
Proof Load must be supplied by a person approved
by project management.
9. OPERATOR LICENSING AND TRAINING
The operator's knowledge of the operating
requirements and safety procedures of the assigned
equipment shall be verified by written examination,
by a hands-on test, and by observation of the
operators performance during the first month of
operation. A competent person designated by the
Project/Site Manager, such as a Superintendent, will
conduct the hands-on test to determine the
applicant's operating ability. A hands-on test will be
conducted on each equipment type and model to be
operated. Training will be provided to each operator
in compliance with Procedure HSE 3.7 Crane and
Equipment Operator Licensing.
Equipment Operator Certification Record must be

completed and retained on file by the Contractor for
the duration of the Project. The operator will be
issued attachment's Equipment Operator's License,
which is to be retained on the operator at all times
while on the Project. This license is valid for the
issuing Project only.

escrito del fabricante. Las etiquetas adhesivas,
rtulos o placas que indican la capacidad,
operacin e instrucciones de mantenimiento
deben cambiarse debidamente despus de
realizar la modificacin.
10.2 Requerimientos para Obtener la
Licencia
La licencia para operar un montacargas se
emitir una vez que el operador haya sido
capacitado / certificado. Cada tipo diferente
de montacargas requerir de una certificacin
individual para su operacin. Una persona debe
proporcionar evidencia de que es competente no
slo en el manejo a nivel general, sino que
adems en el item especfico de maquinaria o
equipo que operar, o en la labor que
desempear.
Cada accesorio de un montacargas requerir de
certificacin para su operacin.
Se requiere una re-certificacin anual, que
consiste en la revisin del material de
capacitacin, normas de operacin, el manual
para el operador entregado por el fabricante y
en la aprobacin de un test de manipulacin.
La persona que recibe la licencia debe leer y
entender el manual para el operador, entregado
10. FORKLIFTS
10.1 Inspection
Prior to operating the forklift for the first time
during a shift, the operator shall inspect the
forklift in accordance with the manufacturers
guidelines.
Forklifts used for day-shift-only operations will
be inspected before use on that day. Prior to
operating the forklift for the first time during a
shift, the operator shall inspect the forklift in
accordance with the manufacturers guidelines.
Forklifts used during shift operations will be
inspected at the beginning of each shift prior to
use.
A documented daily inspection check-list is
required for Forklifts, Tractors, Loaders, and
Excavators and similar by the operator. This
inspection will be performed prior to its use by
the operator.
A qualified inspector/mechanic must inspect all
forklifts prior to their use on site and monthly

kept. Retention is three (3) ears. The person
shown as being responsible for (owning) the
forklift shall keep the records.
Modifications or additions which affect capacity

and safe operation of the forklift shall not be
performed without the manufacturers prior
written approval. Capacity, operation, and
maintenance instruction plates, tags, or decals
shall be changed accordingly following the
modification.
10.2Licensing and Requirements

The license will be issued for the forklift on
which the operator is trained / certified. Each
different "type" of forklift will require individual
certification for its operation. A person must
provide evidence of competency on the
specific item of machinery or equipment they
will be operating or the work activity they will
be undertaking, not just the class or group.

10.3 Capacitacin del Operador
Slo a los operadores capacitados y autorizados
se les permitir operar un vehculo industrial
(montacargas).
El gerente de obra debe
identificar quin recibir la licencia para el
Proyecto. Quienes reciban dicha licencia deben
ser conocedores, experimentados y capaces de
desempear las evaluaciones de competencia.
Los operadores deben ser capacitados por una
persona (s) autorizada, en un proceso que
incluir instruccin en sala de clases (lecturas,
videos o una combinacin de ambos),
capacitacin prctica y una evaluacin. Estos
operadores deben completar exitosamente toda
la capacitacin antes mencionada, adems de
aprobar la evaluacin, antes de que puedan
operar un vehculo industrial sin supervisin
directa. Los operadores que hayan completado
la capacitacin deben ser evaluados mientras
operan el vehculo en el lugar de trabajo. Puede
requerirse de una capacitacin o evaluacin
adicional para mantener una base de habilidades
suficiente para ejecutar las labores asignadas.

Operators License which is to be retained on

Project.
Equipment Operator Certification Record must

The operator must carry his/her Forklift
Operator's Card whenever operating a forklift.
Candidates for a motorized equipment permit
must
meet
the
following
minimum
requirements:
Be mentally alert and physically capable
of driving motorized equipment safely.
Display a responsible attitude.
Be familiar with the motorized equipment
to be operated or be able to adapt
quickly with proper training.
10.3Operator Training
Only trained and authorized operators shall be
permitted to operate a powered industrial truck
(forklift). The Project/Site Manager shall
identify who will be the licensor for the

training and evaluation before they may
operate a powered industrial truck without
direct supervision. Operators who have
completed training must be evaluated while
they operate the vehicle in the workplace.
Additional training or assessment may be
required to maintain a sufficient skill base for
the prescribed duties.

Trainees may only operate a forklift for training

purposes under the direct supervision of the
trainer and then only in an environment which
does not endanger any employee. The trainee
should be assigned to an experienced operator
who will provide practical maneuvering
demonstrations. At this point, the trainee
actually performs basic operations of the
equipment under the close supervision of the
experienced operator. The trainee will be given
demonstrations and the opportunity to practice
operating on each piece of equipment
designated within the skill level category.

10.4Fork Lift Operations

The operator shall not allow arms or legs to be

placed between the uprights of the mast or
outside the running lines of the truck at any
time while in operation.
When an industrial power truck is unattended,
load-engaging means shall be fully lowered,
controls shall be neutralized, power shall be
shut off, and brakes shall be set. Wheels shall
be chocked if the truck is parked on an incline.
A safe distance shall be maintained from the
edge of ramps or platforms while on any
elevated dock, platform, or freight car. Trucks
shall not be used for opening or closing freight
doors.
Brakes shall be set and wheel chocks shall be
in place to prevent movement of trucks, trailers
or railroad cars while loading and unloading.
Fixed jacks are necessary to support a semitrailer during loading or unloading when the
trailer is not coupled to a tractor. Dock plates
are to be inspected to ensure that they are
secure and the load capacity verified.

10.5Forklifts in Hazardous Work Areas
Forklifts used in areas where ignitable gases

10.6Loading and Unloading

Only stable or safely arranged loads shall be
handled. Caution shall be exercised when
handling off-center loads that cannot be

before lifting or traveling.
Only loads within the rated capacity of the truck
shall be handled.
Extreme care shall be used when tilting the load
forward to backward, particularly when high
tiering. Tilting backward with load engaging
means elevated shall be prohibited except to
pick up a load. An elevated load shall not be
tilted forward, except when the load is in a
deposit position over a rack or stack. When
stacking or tiering, only enough backward tilt to
stabilize the load shall be used.
Compressed gas cylinders shall be supported

in an approved rack for transport by forklift.
Laying the cylinders horizontally across the
forks for transport shall not be permitted.
Only loads within the rated capacity of the
forklift shall be handled. Long or high (including
multiple-tiered) loads that may affect capacity
shall be adjusted.
When fork adjustments become necessary to

safely pick up a load, only trained/experienced
personnel familiar with the equipment shall
adjust the width of the forks. When adjusting
forks that are not attached to the mast, the
following steps must be taken to minimize the
risk of possible injury:
Raise the forks at least 1.2 m to place forks in
a more accessible position.
Tilt forks forward 45-degrees to redistribute
the weight to aid in the adjustment.
Properly lubricate the bar.
Use the palm of the hand placed in the
middle of the fork below the 90-degree

agents should be used for cleaning forklifts.

The tugger operator must also visually inspect

equipment daily to ensure safe operation of the
equipment.
Operator Qualifications
Operators must meet requirements equivalent
to those required for crane operators as
outlined in Procedure HSE 3.7 Crane and
Equipment Operator Licensing.
Only qualified personnel may operate air tugger
equipment.
Operations
Procedures and checklists shall be used to
ensure that each set-up of air tugger equipment
is reviewed and approved by a Qualified
Inspector.
Items to be reviewed shall include anchorage,
weight of items to be hoisted/pulled, application
of appropriate coefficients of friction, load
capacity, load obstructions, etc
Loads to be lifted/ pulled are restricted to a
maximum of 75% of the rated capacity of the
tugger and/ inspected daily by the crew
Fforeman and tugger operator prior to
operation. Both employees shall sign off on the
daily inspection log verifying said inspection
Only personnel essential to the operation are
allowed to work near the operating tugger, load
line and the moving load.
Never use the tugger line for other than its
intended purpose (i.e. tie-off for safety
harness).
Special care shall be maintained during
operation of the tugger to ensure the load does
not hang up causing the air tugger to be
overloaded.
A tugger plan should be prepared and
submitted for review and acceptance by the
Project Rigging Consultant prior to operating a
tugger in each new configuration.
