Narrative Summary:

20 MAY 2015: Sector Puget Sound conducted a COC renewal exam for the MODU (option C) NOBLE DISCOVERER, IMO
6608608. PII, ISPS III, and BWM exams were also conducted IAW MSM Vol. II Section D, international laws and treaties, and
applicable U.S. laws and regulations. The NOBLE DISCOVERER is a Liberian flagged, 571.9ft long, 10264 GT, self propelled
drillship with a keel laid date of 1965 and conversion date of 1976. The vessel's class society is DNV-GL. The vessel was
moored at Port of Everett, stbd side to the pier.
In addition to Sector Puget Sound personnel, the COC exam was conducted with Sector Anchorage, OCS NCOE, and CG-5 TI
personnel in attendance. Bureau of Safety and Environmental Enforcement (BSEE) Inspectors attended the exam to familiarize
themselves with CG inspected equipment as BSEE inspectors will be living onboard the vessel 24/7 during drilling operations.
Issued Certificate of Compliance to expire on 20 MAY 2017, Form A, and Form B.
HULL INSPECTION:
Examined the following certificates and documents: IMO MODU Certificate, Stability Letter and Booklet, COFR, Construction
Portfolio, Minimum Safe Manning Certificate, Certificate of Registry, ITC, Cargo Ship Safety Radio Certificate, ISM DOC, ISM
SMC, ILLC, IAPP, EIAPP, IAFS, Class Certificates, IOPP, SOPEP, NTVRP, ORB, Bunkering Notes, EPA Vessel General
Permit, Garbage Log, Waste Management Plan, and BWM Plan; all sat. Verified proof of BW Report submittal. Examined all
officer US licenses with Liberian endorsements and verified compliance with the Minimum Safe Manning Certificate; all sat.
Verified required number of lifeboat certified persons were onboard; all sat. Examined fire and abandon ship drill records; all sat.
Examined maintenance records for lifesaving equipment; found that the servicing report for lifeboats (LB) LB#1: POH4800173153 and LB#3: POH#4500173153 noted unsatisfactory air tank pressure gauges near helm. Chief Mate stated
replacements were onboard. The gauges were installed during examination and examined satisfactorily. Examined firefighting
equipment maintenance records; found that Foam Analysis Report noted failed alcohol fire resistance. USCG and DNV-GL
surveyor deemed Foam Analysis Report satisfactory due to the fact that the vessel does not carry alcohol onboard. Found
Operating Manual dated 04 FEB 2015 complete. Found annual Crane Load Test Statement of Fact Survey report for port and
stbd cranes were dated 16 NOV 2013; issued deficiency #12. Examined report detailing internal, hydrostatic, NDT, and pressure
relief valve testing for all pressure vessels dated FEB 2014; all, sat.
Tested/inspected navigation/bridge equipment, all sat. Witnessed steering test from bridge; all sat. Conducted deck walk:
Inspected deck/hull openings, safety and security items, hull markings, and draft marks; all sat. Examined port crane (Liebherr
MTC 1900-50D) & stbd crane (Liebherr MTC 1900-60D); all sat. Inspected firefighting equipment; paint locker fixed CO2 bottle
appeared to be undersized; issued def #11. Inspected heliport and associated firefighting equipment; all sat.
Inspected lifesaving equipment: Lifeboats were designed, equipped and maintained in accordance with the route and cargo of
the vessel. Completed check of lifeboat breathing air system; found Stbd LB #3 pressure gauge inoperable; corrected on the
spot. Found Port LB #4 leaking air at the manifold fitting; corrected on the spot. This was due to the noted unsatisfactory tank
pressure found as mentioned previously. Checked food, water, & distress flares of all 4 lifeboats in the stowed position; all sat.
Witnessed operation of all 4 lifeboat engines and steering while in the stowed position; all sat. Witnessed operation of port side
lifeboats' sprinkler systems after lowering the lifeboats to the water; all sat. Tested port lifeboat limit switches during retrieval of
lifeboat; all sat. Unable to witness in-water operation of Fast Rescue Boat due to utility barge moored adjacent to launch area;
engine tested satisfactorily in cradle. Starboard side lifeboats not lowered due to vessel being moored starboard side to pier.
Conducted general health and safety walkthrough: Examined hospital and first aid equipment; found excessive boxes stowed in
the hospital space; corrected on the spot. Medical items stocked and inventoried; all sat. Spot checked emergency lighting,
crew accommodations, and workplace health and safety; all sat. Examined galley spaces and noted boxes obstructing freezer
door; corrected on the spot. Found an automatically closing galley fire door with its fusible link located just outside of the galley;
issued def #1. Received letter dated 20 MAY 2015 from the Liberia Maritime Authority granting authorization for the fusible link
to be installed just outside the galley; cleared def #1.
SECURITY:
Conducted ISPS III inspection. Reviewed all vessel records and documents, including International Ship Security Certificate,
security maintenance logs, and security incident procedures. Evaluated communications, restricted areas, access control,
monitoring, and security incident procedures. Vessel found to be in compliance with ISPS Code.
HAZARDOUS LOCATIONS:
Examined hazardous location plan and conducted walkthrough from the drilling floor, throughout mud system, and return. Crew
provided a third party report regarding electrical hazards; all issues identified were minor and had corrective action plan initiated;
all sat.
MACHINERY INSPECTION:
Met with Chief Engineer and conducted operational test of steering gear both locally and remotely in follow up and non-follow up
modes; all sat. Conducted walkthrough of all machinery spaces paying particular attention to fire hazards, shock hazards, and
watertight integrity. Found aft water mist system improperly aligned; issued def#3; corrected on the spot. Found aft water mist
sensor tubing is broken; corrected on the spot. Found several electrical panels that were not latched; corrected on the spot.
Found several emergency lights that were missing/inoperable; issued def #13. Found debris (rust/scale) in the Generator Room
port side vent supply trunk; corrected on the spot. Witnessed test of WTDs @ the following locations: Engine room, port sack
room, port P-tank room fore & aft, stbd #4, & stbd generator room; all sat. Visually examined all accessible fuel supply lines, sea
water piping, and all other associated equipment. Found that remote shut-off valves were missing labels; issued def #4. Found
reach rod for fwd main ballast valve inoperable; issued def#9. Found emergency bilge valve inoperable; issued def #2; corrected
on the spot.
Engine safeties verified:
MDE is a low speed direct-drive diesel, therefore safety testing was conducted w/engine secured.
MDE: Oil Mist Detectors (1, 4 & 'C'); all sat. Low L/O alarm; all sat
MDG 5: attempted reverse power relay (RPR) but procedure was incorrect and test was unsatisfactory; later re-tested
satisfactorily but procedures need to be created with manufacturer's documentation to support the method used; see def #14.
MDG 6: Overspeed shutdown; intake air flaps closed; Low L/O shut-down; E-Stop; all sat.
MDG 4: Overspeed shutdown; intake air flaps closed; all sat.
MDG 1, MDG 2, & MDG3 were not tested due to an ongoing cooling water duplex strainer change-out; issued def # 10.
Strainers are to be replaced on MDGs 4, 5, & 6 after the completion of the first three MGDs.
Emergency Generator:
-Witnessed E-Gen auto-start & bus transfer (2 & 10 seconds, respectively); all sat. Overspeed was attempted but crew did not
know how to conduct the test; issued def #6. Low L/O alarm tested sat. 2nd means of starting failed due to a dead battery bank;
issued def #7. E-Stop tested sat. One of four E-Gen Room fire dampers did not close as required, issued def #5; corrected on
the spot. Tested E-Gen F/O valve remote trip; all sat. Found that CO2 system labeling and remote release instructions were
incomplete/inaccurate; issued def #8. Found E-Gen right bank J/W pre-heater hose damaged due to excessive heat; corrected
on the spot.
Noted crew and technicians onboard were unfamiliar with engine/generator safety device testing; see def #14.
Aux boilers were not tested due to being 'cold iron'.
MARPOL:
Reviewed Oil Record Book procedures with C/E prior to commencing testing. Tested OWS and reviewed OCM data; all sat.
The bunkering of L/O was not entered into the ORB if the L/O was brought onboard via packaged containers (55-gal drums).
Checked F/O samples for MARPOL Annex VI compliance. Noted one sample from Jurong Shipyard on 08 JUL 2014 was present
but did not meet labeling/sealing requirements. Noted that the current Chief Engineer had only been onboard for 4 voyages.
Counseled newly reported Chief Engineer on the importance of documenting packaged containers and the importance of proper
sample labeling and sealing.
SAFETY MANAGEMENT SYSTEM:
Examined vessel's Safety Management System IAW ISM. Personnel possessed heightened awareness of Safety Management.
SMS found to be satisfactory with the following exceptions:
1) SMS procedures lacked procedures to test/maintain the emergency generator and related equipment, including failure of the
secondary battery bank,
2) the crew's inability to conduct an over speed shutdown test,
3) improper labeling of fixed CO2 system protecting the e-gen space, and
4) the fire damper located on emergency generator room bulkhead was unable to properly close. Issued def #14.
DRILLS:
Fire Drill - Initiated within the Smoking Tea Room via actuation of a heat detector. Crewmember selected to find the simulated
fire had a firm grasp of reporting call points & use of portable firefighting equipment. General Alarm sounded and master took
appropriate actions. Crew immediately set fire watches & cooled boundaries, shut down ventilation, & simulated securing
electrical power. Witnessed dress-out of Fire Team 1 but due to rapid response by the primary team and boundary procedures,
the fire was called 'out' upon arrival of Fire Team 1. Flash hoods were not properly donned and discussed the same with the
team/crew. Observed good management/accountability of fire team members during the transit from the fwd Fire Locker to the
accommodations. Tested highest and lowest fire hoses; all sat.
Abandon ship drill - Witnessed satisfactory abandon ship drill with stbd lifeboats simulated to be out of service. Conducted drill
on the port side (combination of primary & alternate stations). Crews were mustered in 4 separate areas to reduce
confusion/congestion. Full muster was obtained but was delayed due to 2 contractors being counted as crew (contractors were
supposed to disembark for the drill). Boats were not loaded for the drill. To test the crew's ability to obtain proper personnel
accountability, one crewman was asked to stay in his stateroom during the mustering portion of the drill. Lifeboat coxswain &
OIM quickly realized a crewmember was missing, ordered a search, & found him in stateroom. Immersion suits were donned by
the crew separately after the drill on the Mess Deck; required some direction in proper donning. Drill was performed
satisfactorily.
Issued 14 deficiencies: 4 corrected on the spot and 10 remain outstanding. 7 of the deficiencies were required to be corrected
prior to departure to satisfaction of Class and/or CG. Examination complete.

26MAY2015: Attended MODU NOBLE DISCOVERER, to clear deficiencies issued on 20 MAY 2015. The following deficiencies
were cleared:
4. Label remote emergency shutdowns for generator service tank, generator settling tank, boiler service tank, and tank to
pumproom purifier as identified in the operations manual IAW 79 MODU CODE/ 9.5.1.1.1.
Permanent labels for remote shutdowns were properly added.
6. The crew could not test the overspeed shutdown on the emergency generator. Provide valid procedure and verify proper
operation IAW 79 MODU CODE/ 4.2.3. Accepted DNV-GL Class Report, 25MAY2015 of overspeed tested on EGEN indicated
a test procedure was presented to the attending surveyor and the overspeed protection function was tested in the emergency
generator engine with satisfactory results. Added procedures to MISLE documentation as "WIM Report (E-Gen testing
procedures)".
7. The secondary battery bank for the emergency generator is not operational. Ensure secondary battery bank is fully
operational; prove proper operation IAW 79 MODU CODE/ 5.4.2. Accepted DNV-GL class report 25MAY2015 that indicated the
batteries for the secondary battery bank were replaced and the emergency generator engine was manually started using both
sets of batteries with satisfactory results.
8. Provide proper labeling and instructions for CO2 remote release system for emergency generator room IAW 79 MODU
CODE/ 9.5.1.1.2.Observed new instructions and labels.
9. The remote reach rod for the forward main ballast valve is inoperable. Prove proper operation of remote closure IAW 79
MODU CODE 8.3.3. Witnessed engineer exercise valve.
10. Unable to test 3 of 6 generators (out of service for raw water strainer installation). Provide report to capture satisfactory
testing of remaining 3 generators (#1, #2, #3) IAW 79 MODU CODE 4.2.3 & 4.2.4
Accepted Class report from DNV-GL for generator testing that indicated safety shutdown functions for auxiliary diesel engines
numbers 1, 2, and 3 (port outboard, port inboard and center) were all tested and found in order. Overspeed, low lubricating oil
pressure and high cooling water temperature functions were tested on all three engines. Added DNV-GL report to MISLE

13. Provide adequate lighting in the aft machinery space. Replace emergency light bulb located at E-light # 16 in aft machinery
space and E-light bulb located adjacent to WTD #8 IAW 79 MODU CODE 5.3.2.1.3.
Verified proper e-lighting in the aft machinery space and verified e-light operating IWO WTD#8.
14. Procedure should be established for all critical equipment to ensure it is maintained in conformity with the relevant rules and
regulations. To resolve, company should provide the USCG and RO a corrective action report that outlines the implementation of
corrective actions, including measures intended to prevent reoccurrence. Objective evidence to support is referenced on Form B
dated 20MAY2015, items 5,6,7, and 8 IAW 09 SOLAS CH IV. The following corrective actions were deemed acceptable:
a. Received/reviewed new main diesel generator shutdown automation testing procedures.
b. Received/reviewed new main engine shutdown automation testing procedures.
c. Received/reviewed new emergency generator shutdown automation testing procedures.
d. Received/reviewed internal email from Chief Mate to company requesting to add the following language to water mist system
maintenance procedures, "Ensure all potable water valves in system are opened and sealed with tags upon completion of
work/testing. Ensure system is placed back in auto/remote."
e. Received copy email from Chief Engineer sent request directly to company to add a highly detailed emergency generator
weekly maintenance, inspection, and test procedure PM within vessel SMS. All, Sat.
f. Received internal email from Chief Mate to company requesting a PM be created for monthly exercising of ballast valves.

18MAY15: COTP Order 38-15 has been cancelled. The vessel submitted a class report attesting to the main engine's fitness for
service. The vessel's cylinder lube oil strainer was clogged; the strainer was dismantled and cleaned by the crew. Another
strainer, as well as the cylinder oil head tank were opened and cleaned. The oil was also changed. There was no main engine
damage reported or observed. Checking the strainer for cleanliness was added to the planned maintenance program to be
performed monthly as well as prior to starting the main engine.

Deficiency 1: Any Ship of 10,000 Gross Tonnage and above shall be fitted with oil filtering equipment complying with paragraph 7
of this regulation. The installed MEPC 107(49) oil filtering equipment failed to process oily bilge water from the vessels bilge
water holing tank No. 28PS with 9.8 m3 oily bilge water remaining in the tank. At each attempt to operate the oil filtering
equipment it failed with error. Ship detained to the satisfaction of the RO/RSO and Coast Guard. 30ac
Deficiency 2: After any survey of the ship had been completed no change shall be made in equipment and other items covered
by the survey. The PSCO identified that the mud pump room bilge high level alarm was inoperable. An expanded exam of the
vessel maintenance history noted a failure on 12 APR 2015 without corrective action by the crew. 17ac

Personnel and Specialist/Temporary Nature of the Work Performed by the Maritime MODU Advisors
The four individuals that will serve as Maritime MODU Advisors are Mr.

employees based in Shells offices in the United Kingdom and Singapore. Specific information on each
of the identified individuals is provided in Enclosure (A).
The following is a brief discussion addressing each of the factors listed in the G-MOC Policy
Letter 02-01:
Job Description Relative to the Unit: The job description for a Maritime MODU Advisor is discussed in
Enclosure (B).
The Period of Time Requested: It is anticipated that the four individuals will perform their specialized
work from approximately June 1, 2015 November 10, 2015 when activities occur in the Chukchi Sea
OCS. This should be considered temporary in nature, for among other reasons, due to the short openwater season in Alaska.
Type of Operations: As discussed in the background section above and in Enclosure (B), the Maritime
MODU Advisors will perform highly specialized services when the MODU is operating on the Alaska
OCS. In short, the Maritime MODU Advisors will be Shells onboard marine assurance representatives
employed specifically to oversee contractor operations to ensure they are conducted in accordance with
applicable regulatory requirements, industry best practices, and Shells internal maritime standards to
ensure safe and environmentally compliant operations. Maritime MODU Advisors are neither necessary
nor normally utilized in MODU operations. They are clearly not necessary for the routine functioning
of the Vessels. They are neither officers nor crew of the Vessels, they will not stand watches, and they
serve no direct industrial purpose (e.g., their duties are not akin to toolpushers, drillers, roustabouts, floor
hands, crane operators. etc.), and they cannot be considered support personnel (e.g. cooks or stewards).1
Degree of Expertise or Training Needed: Each of the individuals is specifically trained and has extensive
experience performing the functions required of a Maritime MODU Advisor in the offshore Alaska
environment. The Maritime MODU Advisors are highly experienced Shell employees that possess a
specialized level of expertise and in-depth knowledge of the maritime regulatory framework, best
maritime operating practices, Shells internal maritime standards, and a mature HSSE culture. These
individuals would report to the Shell Manager of Maritime Assurance through the on board Shell drilling
For example, the Policy Letter states that a weld inspection technician aboard a pipe-lay barge to ensure quality
assurance or operate equipment that requires specialized training is not considered part of the regular crew
complement. Clearly, a weld inspection technician performing services onboard a pipe-lay barge to assure quality
or operate equipment would benefit the operations and mission of the barge. Nevertheless, because the technician is
aboard temporarily to provide specialized services, he/she is not considered part of the regular crew complement.
Another example involves drilling advisors that have been authorized for use on drill ships during specialized ultradeep drilling operations following the Deepwater Horizon incident to ensure for the overall safety of operations.
Specifically, Wellsite Geologists have been approved to supplement the current crew complement and not replace
any of the U.S. citizen crew. These personnel provide specialized oversight, advice, training, and mentoring of the
crew with regard to drilling operations and they do not stand any watches while onboard the Vessels. These
personnel have been approved for use up to one year at a time in the Gulf of Mexico.

attended the MODU NOBLE DISCOVERER at JSL Mega Yard, Tuas,
Singapore at the request of the owner in order to observe and clear PSC deficiencies issued to the vessel in November of 2012
by Sector Anchorage. Met with Captain, Chief Engineer (Contract), and Mr.
(Compliance Director).to discuss the
scope of the examination and the current state of the vessel.
Guided by the form B issued Nov 2012 by Sector Anchorage, the attending inspectors reviewed the vessel statutory certificates,
third party servicing certificates, and logs for accuracy and currency. Additionally, the current DNV survey report from Korea was
available for review as well as the results of the vessel's ISM/ISPS renewal audit. Only four observations were noted on the
auditors report, These items dealt mainly with the fact that the vessel had not been operating since late 2012 so records of drills
etc were not readily available.
Conducted tour of the vessel for familiarization. Ship's crew identified the new rescue boat that was installed during the
Singapore dry-dock and records of the installation tests were provided for review. The four lifeboats on the vessel also had their
releases replaced as per the IMO/USCG requirements. All were satisfactorily tested and documentation of testing provided to the
inspectors for review. All five generators were replaced and testing of the safety shutdowns was conducted in the presence of the
attending inspectors and DNV surveyor. The main engine was replaced in Korea and successful sea trials were conducted in
November of 2014. Initial sea trail was unsuccessful due to improper matching of propeller to hull/engine design. This was
corrected in Singapore and resulted in the successful sea trails being completed in the presence of the DNV surveyor. The SFP
in the engine room has also been replaced. New insulation throughout the engine room was installed as a result of the new
engine installation along with the exhaust system for the main engine. A walkthrough of the machinery spaces also included
inspection of the oily water collection tank that is now included on the vessel's IOPP and documented/approved by DNV. The
OWS was not fully tested in the presence of the attending marine inspectors due to the vessel being in the shipyard and inability
to take suction from the tanks as all bilge water was currently being transferred to shore. However, a cursory inspection of the
OCM including the calibration certificate was deemed satisfactory as well as alarm annunciation in the engine room and via the
Kongsberg system. The attending DNV surveyor did complete a full examination of the OWS during the IOPP renewal survey
and no observations or comments were noted following his inspection. (see attached DNV Survey report). The ships master also
identified numerous new self closing fire doors in the accommodations block and pilot house. These were replaced based on the
findings of the PSC inspection in November of 2012. A random sampling of the doors and their closures were all found to be
satisfactory. Electrical transit connections and MCT locations were inspected for the presence of dead end wires / cables and
proper installation / construction of cable transits. These were found to be in satisfactory order as well. DNV survey reports
documenting these items and their corrective actions is attached to this case for review.
The 09 remaining deficiencies were cleared after satisfactory resolution was presented to the attending inspectors and
consultation with the attending DNV surveyor in addition to discussion with Sector Anchorage. Vessel's intentions are to seek a
renewal MODU COC on the USWC upon its return. Vessel currently conducting work up training with ship's crew off the coast of
Malaysia in preparation for return to operation sometime in late 2015..

Bottom
At the request of the owner's representative, the vessel's bottom survey was carried out during her staying at the
dry-dock and found to be satisfactory. Note that bilge keel p lates on port and starboard sides were cut off and removed
at the owner's decision which will not be affected to the class.
Stability
-. An occasional survey for 'Lightweight survey and Inclining experiment' was carried out in the presence of surveyor
according to the approved procedure (ref. no. TAMUS475/HUYNH/ 27355 -J-10608).
All records and measurement has been confirmed and endorsed by attending surveyor (see separate survey report of
2014-03-09 / job id. N345625-1).
-. As per the approval letter Ref no.MCANO845/JHYUN/ 27355-J-10709 (document number 13-026-NR-10-003), it
was verified that the loading co mputer software was installed on t wo(2) independent computers and the results fro m the
calculations onboard are same as for the approved test conditions. Upon completion of the verification, the loading
