Estudio Comparativo Entre Los Montajes Convencional y Estético
RESUMEN
Introduccin: En prostodoncia para realizar un anlisis integrado y lograr el diagnstico y un plan de tratamiento, es necesario
realizar un montaje del modelo maxilar en
el articulador, transfiriendo la posicin tridimensional de la arcada maxilar con respecto
a referencias crneomandibulares. Para ello
se utiliza el arco facial cinemtico, el cual en
muchas ocasiones transfiere el plano oclusal
al articulador de forma sesgada inducida por
la asimetra de los meatos acsticos externos.
Para ello, se ha desarrollado un montaje de
ndole esttico, en el que se toma en cuanta
la lnea interpupilar como referencia contra
el aditamento que une a los dos brazos del
arco facial, para modificar el arco facial en su
ajuste y anular el efecto de los meatos acsticos externos.
Abstract
Introduction: To realize a diagnosis and
treatment planning in Prosthodontics to
mount the diagnostic casts is mandatory, so
the tridimensional transfer of the maxillary
relationships against the craniomandibular
references, using the cinematic face bow,
which could induce to and error because of
the auricular asymmetries. For that reason
en aesthetic technique has been develop,
which ignores the influence of the ear components and parallels the anterior par of the
face bow to the interpupilar line. Purpose:
To determine if there is a statistically difference between the conventional technique to
apply the face bow versus the aesthetic one.
Materials and methods: A 30 students sample of the State University of Costa Rica with
complete natural permanent dentition were
selected. Two maxillary study casts (one for
each technique) were constructed from type
III dental stone (Whip-Mix) following the
fabricant specifications. The type of technique to used was selected a random way. Finally, the casts were mounted in a semi-adjustable articulator and the distance from selected
teeth (3,9, and 14) to the mounting plate basis
were measured with a digital caliper (Mitutoyo - Japn Digital BH 305) with a 0.001
mm precision. A frequency distribution of
values was achieved for each technique and
dental measurement units. ANOVA analysis
and Bonferroni Test were used for statistic
results. Results and conclusions: no significant statistic difference was found between
the two different techniques at the posterior
teeth. A significant statistic difference was
found at the anterior measure area, revealing that perhaps, the auricular asymmetries
were well amplified and more evident at the
anterior zone.

INTRODUCCIN
La Odontologa, hoy en da, se centra
fundamentalmente en la compresin y manejo
del sistema estomatogntico, como unidad
morfofuncional bien definida, en especial de los
movimientos mandibulares que se ejecutan en
relacin al plano funcional oclusal, el cual debe ser
analizado tanto desde un punto de vista esttico,
como dinmico. (Ramfjord & Ash, 1982).
Entender el funcionamiento normal de la
oclusin es imprescindible, solo as se puede
diagnosticar y tratar las diversas afecciones que
alteran el sistema estomatogntico (Ramfjord &
Ash, 1982).
Para tal anlisis, en Odontologa se acostumbra
a construir modelos de estudio de los arcos
dentarios, a partir de una impresin que es
vaciada con piedra dental tipo III, y mediante
registros interdentales, se obtiene informacin
para orientarlos en aparatos conocidos como
articuladores.
El glosario de trminos prostodnticos (1999),
as como el glosario de operatoria dental (1983)
definen el articulador como un aparato
mecnico que representa las articulaciones
temporomandibulares y los maxilares, al cual
se le pueden colocar los modelos maxilar y
mandibular y realizar algunos o todos los
movimientos mandibulares. Los articuladores
se presentan en diversas formas y tamaos,
varan adems, en complejidad y precios.
En general, los articuladores simulan los
movimientos
mandibulares
del
paciente,
proporcionan relaciones maxilomandibulares
estticas y dinmicas. Para lograr con xito la
transferencia de estas relaciones a dicho aparato
mecnico, se emplea un aditamento llamado arco
facial, el cual ha venido evolucionando en sus
aditamentos y referencias hasta llegar al sistema
actualmente utilizado, llamado cinemtica
(McCollum, 1960; Weinberg, 1963; Dawson,
1974; Ferrario et al, 2002).
El arco facial de montaje rpido es el
instrumento que permite registrar varias de las
referencias anatmicas del paciente, para luego
trasladarlas al articulador semiajustable. Entre
las utilidades principales del arco facial, est
que permite localizar la posicin del modelo
superior en el espacio, con respecto al plano
axio-orbitario. Una vez retirado el arco facial, se
tiene el plano oclusivo tridimensional.
Histricamente se hace el montaje de la rplica
superior utilizando, como referencia para la
orientacin del plano oclusivo e inclinacin
condilar, dos planos arbitrarios (plano de
Frankfort y plano axio-orbitario), se asume
que son paralelos al piso. Se ha notado que esta

Tcnicas de anlisis estadstico: Para el anlisis de los datos, se realiz una distribucin de frecuencias de los valores observados para cada una
de las tcnicas y de las piezas y el clculo de los
estadsticos descriptivos tales como: el promedio
de la altura para todas las piezas por cada modelo
se calcul con la siguiente frmula:

Para evaluar los modelos se plantearon las siguientes hiptesis estadsticas y se probaron con
un nivel de significancia del 5%.
Estas pruebas se realizaron mediante el uso del
anlisis de variancia (ANOVA), la cual se define
como una tcnica en la que la variancia total
de un conjunto de datos se divide en dos o ms
componentes, y cada uno de ellos se asocia con
una fuente especfica de variacin, de manera
que durante el anlisis es posible encontrar la
magnitud con la que contribuye cada una de esas
fuentes en la variacin total (Daniel, 2002).
Para el clculo del ANOVA se tomar como
modelo un experimento factorial con un diseo
completamente aleatorio con efectos fijos de
dos factores, el modelo de montaje ( ), la pieza
dental ( ), la interaccin entre ambos ( ) y la
medicin de la altura xijk , el cual se escribe de
la siguiente forma:

Cuando se encontraron diferencias estadsticamente significativas entre los factores, se
procedi a estimar las verdaderas diferencias
mediante el mtodo de Bonferroni, el cual es
una de las pruebas a posteriori que permiten
determinar las verdaderas diferencias entre
pares.

RESULTADOS
De la lista de 61 estudiantes de sexto ao de la
Facultad de Odontologa de la Universidad de
Costa Rica, en el ao 2002, se encontr que 30
estudiantes con edades entre los 22 y 26 aos tenan denticin total permanente y estaban dispuestos a participar en el estudio.

No se encontr suficiente evidencia estadstica
para rechazar la hiptesis nula que la variabilidad
observada en las mediciones entre piezas y
modelos sean iguales (p = 0,643), lo que indica
que se cumple uno de los supuestos del modelo
factorial que se analiza a continuacin.

DISCUSIN
Los profesionales en Odontologa han sido
advertidos de que las asimetras naturales de
la cabeza pueden producir discrepancia entre
la lnea interpupilar y el eje intercondilar y
provocar inclinaciones poco naturales del modelo
superior, ya que, como sugiere Nicholls (1977), el
registro con el arco facial transfiere la asimetra
del eje de rotacin mandibular al eje simtrico
horizontal del articulador.
En el presente trabajo se hizo una comparacin
de las diferencias de orientacin del modelo
maxilar, mediante el uso de arco facial funcional
y esttico. Se logr determinar que existen
divergencias entre ambos, las cuales se reflejan,
principalmente, a nivel de la regin de los dientes
anteriores.
En esta investigacin se concuerda con Chiche
y Aoshima (1997), quienes exponen que el modelo maestro debe proporcionar al ceramista la misma perspectiva que el clnico ve en el paciente.
El ceramista podra estimar solo el alineamiento
horizontal y vertical del modelo de trabajo basado en puntos tomados del modelo mismo, a menos

Con base en los resultados del estudio piloto
se calcula una nueva muestra con la siguiente
frmula:

CONCLUSIONES.
1. Dentro de las limitaciones de precisin impuestas
por el operador, equipo, y variaciones de pacientes,
se puede determinar que la localizacin de
posiciones anatmicas asimtricas del eje, puede
llevar al uso inexacto de los arcos faciales. Esto
puede resultar en una impropia localizacin del
plano incisal y oclusal.
2. No existe diferencia significativa entre las dos
tcnicas, cuando los registros fueron hechos a
nivel posterior.
3. Se encontr una diferencia significativa entre
ambas tcnicas cuando los registros se hicieron a
nivel de los incisivos, lo cual lleva a concluir que
las asimetras presentes en los conductos auditivos
externos, presentan variaciones que se reflejan
principalmente a nivel de los dientes anteriores,
al usar la tcnica Arco facial funcional.
