D: Pt. attended healthy eating group with 9 other patients, one therapist, and one
student. Pt. showed concurrent emotions with environment, answered questions
when asked and had a flat affect throughout session. Pt volunteered to help make
smoothies for group. He conversed and participated in group discussion with peers
when determining what to put into the smoothie recipe. Pt. stated goal of reducing
carbs and fats in his diet.
A: Pt. shows improvement in willingness to participate as evidenced from previous
groups leaving in the middle of the session and minimal to no interaction with
peers, therapists, and staff members. Pt. stated a goal which shows improvement as
he has not stated a goal during discussions prior. He showed an interest in healthy
eating tips as he was willing to share with the group.
P: Continue with skilled occupational therapy services until goals are met. Continue
with group discussions and peer interaction activities to improve self-esteem and
socialization, structure, relatable, and allows peer interaction.

D: Pt. stated he is being discharged and needs to get a haircut. Pt. stated he is
going to take a shower today as his goal. He was well-kempt, had a flat affect
throughout, spoke when spoken to, and interacted with peers minimally. Pt.
participated in grooming/hygiene group, stayed for the allotted time, and followed
groups expected rules. Pt. asked therapist for floss pick and shower gel. Pt.
participated in group discussion with appropriate comments. Pt. stated that he has
lower self-esteem when he does not look presentable.
A: Pt. shows improvement with social participation skills as evidenced by eye
contact and participation with therapists and peers throughout session. Pt. was
noted to be laughing periodically which indicates improvement with her demeanor
and attitude.
P: Continue with skilled occupational therapy services to work on social skills,
improve self-esteem, and continue participate in group discussions.

D: Pt. interacted with therapist and students frequently, but interacted with peers
minimally. Pt. came to group on time, freshly showered, and appropriate clothing.
She independently picked out own nail polish and painted fingernails.

A: Based on past groups, pt. requires reassurance from therapists and staff
members and is noticed to be fidgeting throughout group. Pt. does, however,
respond appropriately to group discussions through humor.
P: Continue with skilled occupational therapy services to improve social skills, selfesteem, confidence, participating in group discussions, and attendance of groups.
