Collection of Personal Information


The College of Nurses of Ontario (the College) collects the information in the QA Program Learning Plan Form if you have been selected for Practice Assessment
under the general authority of the Regulated Health Professions Act, 1991, S.O. 1991, c. 18, the Nursing Act, 1991, S.O. 1991, c. 32, and its regulations; and the
Colleges by-laws. The College collects the information for the purpose of assessing your continuing competence through its Quality Assurance Program. Appropriate
measures are taken to safeguard the confidentiality of the personal information you provide and all documents become the property of the College.

Goal #1: Activities and Timeframes


How am I going to achieve my goal? Identify a minimum of three (3) learning activites with timeframes/timelines.
In semester two and three I as able to provide care for up to two patients at a time. In pregrad, I will be assigned a full load,
and will get a chance to deal with all other aspects of the client care needs such as morning care, medications, consultation
with OT, PT and communication with family members. Thus the work load of my assignment will help me to push myself to be
more organized and manageable in order to provide safe and effective care based on client needs. For example: Take the vital
signs and to provide morning care to the assigned clients before breakfast. To give clients their morning medication before
ten o'clock, to check the clients charts for any new orders, laboratory result and their appointments for the day after the start
of the shift, will help me to keep organized for the day.

Learning Goal #2
My goal must be based on my learning needs.
To practice various hands on skills such as wound dressings, catheterization
and to learn to deal with the intravenous medication pump at the end of my
pregrad placement.

Goal #2: Activities and Timeframes


How am I going to achieve my goal? Identify a minimum of three (3) learning activites with timeframes/timelines.
My practice setting has post operative patients with IV, this would give me great opportunity to practice and refine my skills in priming the
the IV lines and giving the medications under the supervision of my preceptor. Also it will make me competent in preforming such skills
before the end of the semester. By having a full load of patients allowed me to improve my communication skills with all levels in the
care environment, not only was this verbal but also written. Another learning activity was the ability to learn and use the various types of
dressings and the effects of each dressing on the wounds.
