Maintenance in simple words is a set of activities which help
keep plant, eqipment, machiner!, vehicles and other
facilities in "ood condition#
Every manufacturing organisation needs maintenance because,
machines breakdowns, parts wear out and buildings deteriorate
over a period of time of use.
Maintenance is an important factor in quality assurance.
nconsistencies in e!uipments lead to variability in product
characteristics and result in defective parts that fail to meet the
established speci"cations.
#ood maintenance management is important for the companys
cost control.
$rgani%ations like airlines and oil re$neries have huge
investments in the e!uipment. %qipment failre will be
disastrous for such companies.
Let us take a look at 5 such incidents caused due to poor
maintenance, and the disastrous impact it can cause not just
to the companies involved but also to the life of many
people.

leakage of a poisonous gas Methyl socyanate
known as M/ into the atmosphere from a
storage tank in a pesticide manufacturing plant.

measures, due to which the leakage was not

due to the uncontrolled leakage of M/. .he
plant was owned and run by 6nion /arbide, nc.

M/ 8Methyl socyanate9 is a lethal substance
that is highly volatile, reactive and to:ic. 'ow
that the chemical is volatile, its fumes start
spreading across the area along with the
prevailing winds.
t was concluded after the investigation that the
traces of water might have entered the M/
storage tank and caused the chemical reaction
which caused the leakage of this highly to:ic
chemical. )ue to the poor maintenance of plant
and ignored security measures, the leakage was
not detected and the chemical continue to leak
into atmosphere.
;ll the security devices including cooling
system, vent gas scrubber, <are tower etc. failed
to operate during the accident due to their poor
design and poor maintenance and
malfunctioning of neutrali%ation facilities.

of Potters -ar railway station. ;s the rear coach
travelled over the points, they moved, causing
the rear bogie of the carriage to cross onto the
ad3acent line and ultimately derail, <ipping it
into the air.
.he momentum carried the carriage into the
station, where one end of the carriage struck a
bridge parapet, sending debris onto the
road below. .he front three coaches remained
upright, and came to a stop to the north of the
station.
.he =ealth and Eafety E:ecutive 8=EE9 report
released in May 211& found that the points were
poorly maintained and that this was the
principal cause of the accident.
.he bolts that held the stretcher bars that keep
the rails apart had come loose or gone missing,
resulting in the points moving while the train

malfunctioned and started moving upwards.

student who lived in the /ity =eights .akeshiba
public housing comple: in the Minato Bard, one
of 2& Bards in .okyo, Aapan. n Aune 211@,
chikawa was backing his bicycle out of an
elevator when the elevator suddenly
malfunctioned.
chikawa was half(way out of the elevator when
it suddenly shot upwards, with the doors fully
open, pinning him against the door frame. =e
was killed by asphy:iation.
Police arrived and eventually pulled chikawa
out, and as they did, even though power to the
elevator had been disconnected, the elevator
still shot to the top story of the elevator shaft,
stopping only at a collision prevention device.
.he investigation found ,1 elevator(related
mechanical problems in two Echindler elevators
installed in the apartment comple: over a two(
year time period. .he "nal cause of the accident

was placed on the failure of a braking system in
the elevator, combined with poor maintenance.
.he Minato Bard public housing corporation
decided to replace all "ve Echindler elevators in
the condominium with elevators from another
manufacturer.

lights, crashed through a perimeter fence,
crossed a roadway and came to rest on a beam.

.he investigation revealed that prior to the
accident the aircraft was operated while the
main landing gear tires were severely
underin<ated because of #lobal E:ec ;viation5s
inade!uate maintenance. .he under in<ation
compromised the integrity of the tires, which led
to the failure of all four of the airplane5s main
landing gear tires during the takeo4 roll.
Ehortly after the "rst tire failed, which occurred
about 1.0 seconds after the airplane passed the
ma:imum speed at which the takeo4 attempt
could be safely aborted, the "rst oFcer
indicated that the takeo4 should be continued
but the captain decided to re3ect the takeo4 and
deployed the airplane5s thrust reversers.
