CIty and FLORIANNE C. QUIJADA, 49 years of age, married, Filipino, and
presently residing at, B1 L35 Valencia street, La Casa Nueva, San Agustin, San
Fernando Pampanga, having been duly sworn to in accordance with law, do
hereby depose and say:
1. That We have personally known the person of FELIX M. LUZANO for a long
period of time;
2. That FELIX M. LUZANO died single on June 12, 2009 leaving no spouse and
known children;
3. That he had designated during his lifetime as beneficiary in his Social Security
System (S.S.S.) the person of PHILIP WILLIAM C. ALTARES;
4. That We recognized said designation to PHILIP WILLIAM C. ALTARES as
beneficiary;
5. That We execute this affidavit for whatever legal intents and purposes it may
serve.
