SHEPHERD UNIVERSITY INTRODUCTION TO THERAPEUTIC RECREATION WRITTEN CARE PLAN and REFLECTION COMMENTS FOR MARK

HEALTH OVERVIEW: Mark is a male about 35 years of age. He has been diagnosed with Lowes syndrome, and mental retardation. Lowes has left him with mobility problems. He has a metal rod in his back and his knee is almost complete turned around because of Lowes. Mark can be very shy and doesnt like to communicate that much. Mark begins the supervised exercise program at The Wellness Center of Shepherd University. MISSION STATEMENT: Mark will be encouraged to engage in new forms of recreation and physical activity while participating at the Shepherd University recreation program. During the next six weeks he will be provided the opportunity to participate in regiment exercise programs and will also experience social interactions with other participants and exposure to information regarding the importance of therapeutic recreation in improving his overall quality of life. This program is to be completed by November 11, 2011. CLIENTS STRENGHTS INCLUDE: 1. The client is aware of his environment and executes appropriately within the Shepherd University Wellness Center. 2. His physical condition makes participation a struggling task. He has really never been able to do much physical activity. 3. The client shows general respect for his surroundings and social situations. CLIENTS NEEDS/CHALLENGES/WEAKNESS INCLUDES: 1. The client has little to no mobility because of his conditions. 2. Mark is easily distracted and lack motivation. He doesnt want to do much other than sit there. 3. Because of his condition, the weather affects how mobile he can be.

PROGRAM SKILL DEVELOPMENT STATEMENT:

PHYSICAL SKILL DEVELOPMENTGOAL- Mark will be encouraged to improve his physical condition OBJECTIVES 1. 2. The client will engage in 10-15 minutes of exercise 3 times a week. The client will improve his total fitness by performing 20 minutes of cardiovascular work-outs a week. 3. To maintain his physical condition the client will be encourage to participate in ADL activities daily.

SOCIAL SKILL DEVELOPMENTGOAL- Mark will engage in socialization related to recreation. OBJECTIVES1. 2. In order to improve socialization the client will engage in light conversation with his peers. The client will participate in at least two hobbies in order to further develop his social skills. These hobbies will include movie discussion, and arts and crafts.

COGNITIVE SKILL DEVELOPMENTGOAL- Mark will improve his recreation knowledge from exposure to new activities. OBJECTIVES1. The client will be taught/learn the repetition of regimented exercises to improve his mobility and ROM. 2. Over the next six weeks the client will participate in mind stimulating activities to improve his cognitive aspects of rehabilitation.

REFLECTION OF MARK Mark came to the program because his care giver was a supporter of it. He cant do much and has problems with mobile tasks such as walking. Mark has Lowes syndrome which causes some degree of mental retardation along with cataracts in his eyes which caused one eye to be removed and the other eye is considered legally blind. Marks condition also made him have major mobility issues, he has a metal rod running the length of his spine and his right knee is nearly turned entirely around. Mark was a tough client and with the lack of mobility and blindness wasnt able to do much. The biggest accomplishment with him was the fact that he became more comfortable around Garrett and me and was willing to talk and have a bit of conversation with us.
