Please bring the following documents:

Proof that you were in the US on 6/15/12 Proof that you have lived in the US since June 2007. Proof that you are currently enrolled in school, OR have graduated from high school, OR have

obtained a GED, OR have honorable discharge. Proof that you entered the US before you turned 16 years old. If you have ever been arrested or cited, you must bring all police and court records. Proof may include school, employment, medical, financial &/or military records.

Favor de traer los siguientes documentos:

Prueba de que se encontraba en los EE.UU. el 15 de junio del 2012 Prueba de que usted ha vivido en los EE.UU. desde junio de 2007 Prueba de que usted est actualmente inscrito en la escuela, o haberse graduado de la

preparatoria (high school), o haber obtenido un GED, o fue honorablemente descargado del ejrcito. Prueba de que usted entr a los EE.UU. antes de cumplir 16 aos de edad. Si ha sido arrestado o citado, traiga sus documentos de la policia y la corte. Pruebas pueden incluir documentos de escuela, empleo, mdicos, financieros o militares.

El solicitante debe estar presente para la consulta.
